IntroductionSarcopenia is a condition diagnosed when the patient presents low muscle mass, plus low muscle strength or low physical performance. Muscle weakness in the oldest (dynapenia) is a major public health concern because it predicts future all-cause mortality and is associated with falls, disability, cardiovascular mortality and morbidity. Grip strength is a simple method for assessment of muscle function in clinical practice.ObjectiveTo estimate the grip strength and identify factors associated with handgrip strength variation in elderly people with low socioeconomic status.MethodsCross-sectional study based on a multidimensional assessment of primary care users that were 60 years or older. The sample size was calculated using an estimated prevalence of depression in older adults of 20%. A kappa coefficient of 0.6 with a 95% confidence interval was used to generate a conservative sample size of 180 individuals. Procedures: tests and scales to assess humor, cognition (MMSE), basic (ADL) and instrumental activities (IADL) of daily living, mobility (Timed Up and Go), strength, height, Body Mass Index (BMI) and social support were applied. Questions about falls, chronic diseases and self-rated health (SRH) were also included. Statistical Analysis: Mean, standard deviation and statistical tests were used to compare grip strength means by demographic and health factors. A multivariate linear model was used to explain the relationship of the predictors with grip strength.ResultsThe group was composed predominantly by women (73%) with a very low level of education (mean 3 years of schooling), mean age of 73.09 (± 7.05) years old, good mobility and without IADL impairment. Mean grip strength of male and female were 31.86Kg (SD 5.55) and 21.69Kg (SD 4.48) [p- 0.0001], respectively. Low grip strength was present in 27.7% of women and 39.6% of men. As expected, men and younger participants had higher grip strength than women and older individuals. In the adjusted model, age (p- 0.03), female sex (p- 0.0001), mobility (p- 0.05), height (p- 0.03) and depression (p- 0.03) were independently associated with low grip strength. For every second more in the mobility test, there was a mean decrease of 0.08 Kg in the grip strength. Elders with depression had a mean reduction of 1.74Kg in the grip strength in relation to those in the comparison groups. There was an average reduction of 8.36Kg in the grip strength of elderly females relative to males. For each year of age after 60 years, it was expected an average reduction of 0.11 Kg in the grip strength.Conclusionour results suggest that low grip strength is associated with age, female sex, height, depression and mobility problems in poor elderly. Grip strength can be a simple, quick and inexpensive means of stratifying elders’ risk of sarcopenia in the primary care setting. Efforts should be made to recognize weaker persons and the conditions associated to low grip strength in order to target early interventions to prevent frailty and disability.

Resumo: O objetivo foi examinar as caraterísticas psicométricas de testes de triagem para idosos e propor um roteiro para a atenção primária. Etapas: (1) confiabilidade interaferidores para testes de desempenho e perguntas de autoavaliação para 8 funções; (2) sensibilidade e especificidade de questões para depressão e apoio social; (3) encontro de especialistas para seleção das atividades instrumentais da vida diária (AIVD); (4) elaboração do roteiro. A triagem durou 16 minutos. A confiabilidade interaferidores para os testes de desempenho foi excelente, mas pobre para perguntas. Depressão e apoio social apresentaram sensibilidade e especificidade satisfatórias (0,74/0,77 e 0,77/0,96). Quatro AIVD foram selecionadas por mais de 55% dos especialistas. Após os resultados, elaborou-se um roteiro que priorizou o uso de testes de desempenho, mantendo questões para humor, apoio social e AIVD. O estudo sugere maior reprodutibilidade de testes de performance em relação a perguntas. Para humor e apoio social, as questões podem constituir uma primeira etapa de triagem. O roteiro proposto possibilita o rápido rastreamento de problemas.

Este estudo de natureza descritivo exploratória objetivou identificar as áreas de ocupação prejudicadas no cotidiano de cuidadores familiares, analisar a autopercepção do cuidador sobre o seu papel e o cuidado prestado, identificar os sentimentos relacionados ao cuidado e conhecer as razões que o levaram a se tornar cuidador principal. A amostra do estudo foi composta por quatro cuidadores participantes do curso para cuidadores oferecido pela Liga Acadêmica de Gerontologia e Geriatria da Universidade de Brasília (LAGGUnB). Para a coleta de dados utilizou-se como instrumento de pesquisa uma entrevista semiestruturada com perguntas sobre o perfil sócio demográfico do idoso e do cuidador, a autopercepção do cuidador em relação ao cuidado prestado, a rotina diária antes e após se tornar cuidador. A análise dos dados foi feita através da análise de conteúdo e emergiram três categorias: rotina do cuidador, identificando o cuidado prestado e autopercepção do cuidado. Dentre os resultados encontrados notou-se que seis áreas de ocupação estão afetadas. Verificou-se também que não houve uma escolha em se tornar cuidador além disto, sentimentos positivos foram evidenciados ao justificar a escolha em exercer o papel de cuidar. Conclui-se, portanto, que foram pontuadas mudanças no cotidiano, atividades foram negligenciadas ou diminuídas. Sendo assim, a participação do profissional de Terapia Ocupacional tem sua intervenção com foco na reorganização da rotina, resgatando a realização de atividades significativas.Palavras chaves: Cuidador, Terapia Ocupacional, Envelhecimento. AbstractThis study had as objectives to identify the impaired occupations in the daily routine of family caregivers, to analyze the caregiver's selfperception about his / her role and care, to identify the feelings related to care and to know the reasons that led him to become a primary caregiver . It is a descriptive and exploratory study with a qualitative approach to the data. The sample consisted of four caregivers participating in the course for caregivers offered by the Academic League of Gerontology and Geriatrics of a public federal university. To collect data, a semi-structured interview was used as a research tool with questions about the socio-demographic profile of the elderly and the caregiver, the self-perception of the caregiver in relation to the care provided, the daily routine before and after becoming a caregiver. The analysis of the data was done through the technique of content analysis and emerged three categories: caregiver routine, care provided and self-perception of care. Among the results found, there were affected occupations: activities of daily living (ADL), instrumental activities of daily living (AIVD), sleep, leisure, social participation and work. It was concluded, therefore, that changes were scored in daily life, activities were neglected or diminished. Thus, the participation of the Occupational Therapy professional has its intervention focused on the reorganization of the routine, rescuing the performance of significant activities.
