Purpose: To determine whether older cochlear implant (CI) listeners differ from younger CI listeners on measures of speech understanding, music perception, and health-related quality of life (HRQoL). In the study, the authors hypothesized that speech recognition would be more difficult for older adults, especially in noisy conditions. Performance on music perception was expected to be lower for older implanted listeners. No differences between age groups were expected on HRQoL. Method: Twenty older (>60 years) and 20 younger (<60 years) implanted adults participated. Speech understanding was assessed using words and sentences presented in quiet, and sentences presented at +15, +10, and +5 dB signal-to-noise ratio conditions. Music perception was tested using the University of Washington Clinical Assessment of Music, and HRQoL was measured using the Njimegen CI survey. Results: Speech understanding was significantly lower for the older compared with the younger group in all conditions. Older implanted adults showed lower performance on music perception compared with younger implanted adults on 1 of 3 subtests. Older adults reported lower HRQoL benefit than younger adults on 3 of 6 subdomains. Conclusion: Data indicate that older CI listeners performed more poorly than younger CI listeners, although group differences appear to be task specific.

Cochl ear implantation is a safe and reliable method for auditory restoration in patients with severe to profound hearing loss. Objective:To describe the surgical complications of cochlear implantation. Materials and Methods:Information from 591 consecutive multichannel cochlear implant surgeries were retrospectively analyzed. All patients were followed-up for at least one year. Forty-one patients were excluded because of missing data, follow-up loss or middle fossa approach.Results: Of 550 cochlear implantation analyzed, 341 were performed in children or adolescents, and 209 in adults. The mean hearing loss time was 6.3 ± 6.7 years for prelingual loss and 12.1 ± 11.6 years for postlingual. Mean follow-up was 3.9 ± 2.8 years. Major complications occurred in 8.9% and minor in 7.8%. Problems during electrode insertion (3.8%) were the most frequent major complication followed by flap dehiscence (1.4%). Temporary facial palsy (2.2%), canal-wall lesion (2.2%) and tympanic membrane lesion (1.8%) were the more frequent minor complications. No death occurred. Conclusion:There was a low rate of surgical complications, most of them been successfully managed. These results confirm that cochlear implant is a safe surgery and most surgical complications can be managed with conservative measures or minimal intervention. Nowadays, cochlear implant has provided a new lifestyle to many hearing-impaired children and adults. Centers which are conducted cochlear implant which is currently 38 in our country and hundreds of cochlear implant practices are being implemented in these centers. Nurses have an important responsibility of providing improvements in patients undergoing cochlear implant surgery, prevention, early detection and management of complications which may be occured. Nurses have needed to educate themselves to be prepared for such technological surgery and to provide the care of patients with the best conditions. The number of medical publications and studies about the implementation of cochlear implant has been increased rapidly, and also in the literature, studies dealing with the nursing aspect of this issue have not been found. In this article, cochlear implant's technical specifications, selection of patients, the impact on quality of life, preoperative and postoperative care of cochlear implant patients with the surgical technique and possible complications are included. This study is expected to contribute to our colleagues working in these units which cochlear implant is implemented and planned to be implemented.
