Interleukin-18, CRP and procalcitonin levels in vesicoureteral reflux and reflux nephropathy

Background: Some patients with vesicoureteral reflux (VUR) develop reflux nephropathy (RN) and a number of them progress to chronic kidney disease (CKD). However, it is unclear to predict which patient will develop RN and/or CKD. The aim of this study is to evaluate the role of Interleukin-18 (IL-18), C-reactive protein (CRP) and procalcitonin (PCT) as an indicator of RN in VUR. Methods: Ninety-three children aged 3.5-16 years with primary VUR were enrolled. Patients were divided into two groups according to the presence of renal scarring (RS). CRP, PCT, blood urea nitrogen (BUN), serum creatinine (Scr), urinary protein (Up), creatinine (Ucr) and microalbumin (Umalb), serum and urine IL-18 levels were determined during urinary tract infection (UTI) free episode. Results: BUN, Scr, Up/Ucr and Umalb/Ucr concentrations were higher whereas calculated creatinine clearance (Ccr) values were lower in RS (þ) group compared to RS (À) group. CRP, PCT, serum and urine IL-18 levels and mean urine IL-18/Cr concentrations were similar in both groups. Serum and urine IL-18 levels did not differ according to the grade of VUR. No significant correlation was found between CRP, PCT and IL-18. Conclusions: Proteinuria and microalbuminuria are valuable hallmarks of RN. CRP and PCT seem not to be reliable indicators of RN in VUR patients. Moreover, serum and urine IL-18 might not predict RN.

Bu çalışmada, son dönem böbrek yetmezliği (SDBY) nedeniyle diyaliz uygulanan hastalarda inflamasyon belirteci olarak prokalsitoninin (PRC), ateroskleroz ve malnutrisyon ile ilişkisini değerlendirmeyi amaçladık. Çalışmaya, SDBY nedeniyle haftada 3 kez hemodiyaliz (HD) tedavisi uygulanan 14'ü kadın 22'si erkek ve periton diyalizi (PD) uygulayan 13'ü kadın 25'i erkek hasta dahil edildi. Hastalar, ateroskleroz ve malnutrisyon varlığına göre gruplandırıldı. Hasta grupları ile inflamasyon belirteçleri arasındaki ilişki istatistiksel olarak analiz edildi. Çalışmaya alınan HD ve PD hastalarının yaş ortalaması sırasıyla 59,3±11,3 ve 50,7±14,9 yıl idi. Ateroskleroz ve malnutrisyon olan HD hastalarında PRC düzeyleri daha yüksek bulundu (sırasıyla, p<0,001 ve p=0,036). HD hastalarında PRC ile CRP (c-reaktif protein) arasında kuvvetli pozitif bir korelasyon gözlendi (p<0,001, r=0,62). PD hastalarında aterosklerozu olanlarda PRC düzeyleri istatistiksel olarak daha yüksek iken malnutrisyon olanlarda anlamlı fark bulunmadı (sırasıyla, p<0,001 ve p=0, 0,414). Ayrıca PD hastalarında PRC ile CRP arasında korelasyon gözlenmedi (p=0,362, r=-0,16). Diyaliz hastalarında, hem ateroskleroz riskinin hem de malnutrisyonun artmasında inflamasyon önemli bir rol oynamaktadır. PRC ve CRP düzeylerin kombine edilmesi diyaliz hastalarında kronik inflamasyonun güvenilir bir belirteci olacaktır.

Procalcitonin serum levels in children undergoing chronic haemodialysis

Infections account for considerable morbidity and mortality in patients requiring haemodialysis (HD). Procalcitonin (PCT)-a low molecular weight protein of 13 kDa-helps one to distinguish viral from bacterial infections and to evaluate the severity of bacterial infections. We investigated (1) PCT baseline levels in eight children undergoing chronic HD with high-flux membranes and (2) changes in the serum levels of PCT, C-reactive protein (CRP) and beta-2-microglobulin (beta2-MG)-a peptide with biochemical characteristics similar to those of PCT-before and after haemodialysis sessions. Blood sampling was performed three times in the mid-week session. Serum PCT of the seven uninfected children before HD sessions was increased (0.75+/-0.07 ng/ml), whereas CRP levels were normal. PCT after dialysis decreased significantly by 40% (P<0.0001) compared with initial values, whereas CRP levels before and after HD were not different. beta2-MG decreased by 70%, probably due to different biochemical properties of both proteins. PCT serum levels 15 min and 60 min after the HD session remained unchanged in comparison with those at the end of the HD session, suggesting accumulation of PCT between HD sessions rather than HD-induced production to be responsible for the increased baseline PCT serum levels. We concluded that CRP serum levels were not affected by HD in our group. Moderately elevated baseline PCT serum levels that are presumably due to reduced renal clearance and uraemia and dialysis-ability of PCT should be taken into consideration. However, increase of serum PCT in patients with severe bacterial infections is generally massive (10-fold to 1,000-fold), suggesting a low risk for false negative results in such cases.

Abstract: The combination of elevated IL-6 and CRP levels was associated with an altered nutritional status. The concomitant elevations in PCT, CRP, and IL-6 could be more sensitive in the evaluation of inflammation.
