Background Published studies have shown contradictory results regarding the relationship between somatometric parameters and varicoceles. We performed a systematic review and meta-analysis to investigate the possible effects of age, height, weight, and body mass index (BMI) on the presence and severity of varicoceles. Methods Databases including EMBASE, MEDLINE, PubMed, Cochrane Library, China National Knowledge Infrastructure (CNKI), Web of Science, and Google Scholar were systematically searched to identify relevant articles published up to March 2020. Two researchers independently identified eligible articles and extracted data. Cochran’s Q statistic and I2 statistics were used to assess heterogeneity. Meta-analysis was performed using StataSE 12.0 software (StataCorp LP, USA). Random-effects models were used to obtain the weighted mean differences (WMDs) and 95% confidence intervals (CIs). Publication bias was assessed using Begg’s funnel plot and Egger’s regression test. Results The search strategy produced 272 articles, of which 18 articles were eligible according to the inclusion/exclusion criteria. A total of 56,325 patients with varicocele and 1,334,694 patients without varicocele were included in the meta-analysis to evaluate the effect of somatometric parameters on the presence and severity of varicocele. The overall results demonstrated that the presence of varicoceles was significantly associated with height (WMD = 1.41, 95% CI = 1.07 to 1.74, P < 0.001) and inversely correlated with BMI (WMD = − 1.35, 95% CI = -1.67 to − 1.03, P < 0.001) but not with age (WMD = -0.93, 95% CI = -2.19 to 0.33, P = 0.149) or weight (WMD = 0.24, 95% CI = -2.24 to 2.72, P = 0.850). The severity of varicocele was inversely correlated with increased BMI but not with age. Conclusion The presence of varicoceles was significantly associated with height and inversely correlated with BMI.

Background Varicocele is the most common correctable cause of infertility in males. Objectives: This study is designed to evaluate the correlation between occupational and physical activities with 6 incidence and severity of varicocele. Methods: This cross-sectional study was performed on patients with varicocele in a tertiary referral center in Iran. The International Physical Activity Score Questionnaire (IPAQ) was utilized to evaluate physical activity. Furthermore, the occupational status was assessed using 10 a standard checklist created by an occupational medicine specialist. Results: 169 patients with a mean age of 29.1 ± 7.3 years were enrolled. Grade 1 varicocele was found in 49.7% of the patients, followed by grade 2 (47.9%), and grade 3 (2.4%). There was no association between varicocele grade and fertility status, diabetes, hypertension, and smoking. 14 There was a significant difference in patients' varicocele grade between low, moderate, and high15 intensity occupations, indicating that the majority of patients with low and moderate-intensity occupations had grade 1 varicocele, whereas most of the patients with high-intensity occupations had grade 2 varicocele. There was a negative correlation between varicocele grade and exercise. 18 On the other hand, there were positive correlations between varicocele grade and squatting, pulling 19 objects, and climbing stairs. Conclusion: The intensity of occupational activity was associated with the severity of varicocele, and patients with high-intensity occupations had higher grades of varicocele. Physical activities such as squatting, pulling objects, and climbing stairs increase the grade of varicocele, while exercise reduces the grade of varicocele.

Objectives: To assess the role of varicocele grade on seminal fluid parameters improvement post varicocelectomy. Methods: This retrospective study was conducted at Prince Hussein Urology and Organ Transplant Center from the period of January 2011 to December 2014. Two hundred and seventy five male patients with clinical varicocele who underwent varicocele surgery for abnormal seminal fluid parameters were involved in the study. Forty five patients had grade I varicocele, 132 patients had grade II varicocele and 98 patients had grade III varicocele. Two seminal fluids analysis tests were obtained from each patient preoperatively and another two (3-6) months postoperatively to assess the improvement in both sperm count and total sperm motility postvaricocelectomy. Results: The sperm count improved in all three grades of varicocele postoperatively. Grade III varicocele patients had the highest improvement from (21.5± 13.8 million/ml) to (39.8 ± 10.2million/ml), P value <0.05 in 81% of patients. The mean sperm count improvement for grade I , II and III varicocele was 10.1, 12.3 and 13.5 million/ml respectively. Also the total sperm motility improvement was the highest in patients with grade III varicocele from (34.5± 13.6 %) to (55.3 ± 11.9 %), P value <0.05 in 83% of patients. The mean sperm motility improvement postvaricocelectomy was 14.2% , 18.1% and 23.6% for grade I, II and III varicocele respectively. Conclusion: The grade of varicocele has a direct impact on sperm parameters improvements post varicocelectomy with grade III varicoceles had the greatest improvement.
