Objectives To evaluate the accuracy of integrated models (IMs) constructed by pretreatment cone-beam computed tomography (pre-CBCT) in diagnosing alveolar defects after treatment with clear aligners. Materials and Methods Pre-CBCT and posttreatment cone-beam computed tomography (CBCT) scans from 69 patients who completed nonextraction treatment with clear aligners were collected. The IMs comprised anterior teeth in predicted positions and alveolar bone from pre-CBCT scans. The accuracy of the IMs for identifying dehiscences or fenestrations was evaluated by comparing the means of the defect volumes, absolute mean differences, and Pearson correlation coefficients with those measured from post-CBCT scans. Defect prediction accuracy was assessed by sensitivity, specificity, positive predictive values, and negative predictive values. Factors possibly affecting changes in mandibular alveolar defects were analyzed using a mixed linear model. Results The IM measurements showed mean deviations of 2.82 ± 9.99 mm3 for fenestrations and 3.67 ± 9.93 mm3 for dehiscences. The absolute mean differences were 4.50 ± 9.35 mm3 for fenestrations and 5.17 ± 9.24 mm3 for dehiscences. The specificities of the IMs were higher than 0.8, whereas the sensitivities were both lower (fenestration = 0.41; dehiscence = 0.53). The positive predictive values were unacceptable (fenestration = 0.52; dehiscence = 0.62), and the overall reliability was low (<0.80). Molar distalization and proclination were positively correlated with significant increases in alveolar defects at the mandibular incisors after treatment. Conclusions Alveolar defects after clear aligner treatment cannot be simulated accurately by IMs constructed from pre-CBCT. Caution should be taken in the treatment of crowding with proclination and molar distalization for the safety of alveolar bone at the mandibular incisors.

Introducción: La mandíbula en el sector anterior es un área que presenta problemas al momento de realizar diagnósticos y planes de tratamientos. Esto se debe a que las dimensiones de la tabla vestibular de los incisivos mandibulares son muy pequeñas en comparación con la tabla lingual. La aplicación de CBCT puede proporcionar información crucial en el estudio de la morfología de la cortical ósea mandibular. Objetivo: Determinar el análisis morfogeométrico de la estructura cortical de los incisivos centrales mandibulares mediante TCCB. Materiales y métodos: estudio de tipo imageneológico, descriptivo, observacional no experimental de corte Transversal. La muestra está conformada por 60 tomografías obtenida de una base de datos de 168 pacientes Resultados: Se analizó la dimensión de las corticales óseas de la pieza 31 cuyos resultados dieron que la cortical bucal midió aproximadamente 2,03 mm y la cortical lingual 2,43mm así mismo en la pieza 41 la cortical bucal midió 1,97mm y la cortical lingual 2,51mm. Se estableció una densidad ósea presente en la cortical de los incisivos centrales mandibulares mediante Índice de Misch cuyos resultados fueron: D1: 0%, D2:10%, D3: 78%, D4: 17%. Se proyectó como resultado, en la pieza 31 un 57% de dehiscencias óseas y 43% de ausencias, en la pieza 41 un 60% de dehiscencias óseas y 40% de ausencias. Conclusión: Se determinó que la cortical lingual es de mayor tamaño que la cortical vestibular encontrando una diferencia de 0,99 mm entre las corticales. También pudimos identificar que hubo una prevalencia en la densidad ósea de un 73% de hueso tipo D3, determinando también la altura de la tabla vestibular con un resultado aproximado de 7, 55 mm en la pieza 31 y en la pieza 41 dando un promedio aproximado de 8,21 mm, consideramos la presencia de dehiscencias de la cortical vestibular ósea dándonos como resultado más de un 50% de dehiscencias encontradas en la cortical vestibular de los incisivos centrales mandibulares.

Background The vertical facial growth pattern is one of the most important issue in the orthodontic diagnosis and treatment. Previous studies investigated the association between interdental bone thickness and facial divergence using mainly bidimensional analysis. When two-dimensional dental radiographic views are not sufficient for diagnosis and measurements, cone-beam computed tomography (CBCT) images should be used to assess the alveolar bone structure three-dimensionally and with high accuracy and reliability. The aim of the present study was to evaluate the correlation between alveolar bone thickness and facial divergence in young adults untreated patients using a three-dimensional method analysis with CBCT images. Methods Records of 30 untreated patients (mean age 16 ± 2 years) with Angle Class I and mild to moderate crowding were analyzed. Subjects were classified as hypodivergent (<39˚), normodivergent (41 ± 2˚), and hyperdivergent (>43°). according to the inter-maxillary angle between the sagittal maxillary plane (ANS-PNS) and the mandibular plane (GN-ME). The alveolar bone thickness measurements were taken for the buccal and palatal/lingual surfaces of maxillary and mandibular anterior teeth. Axial-guided navigation (AGN) was used to locate all landmarks using a specific software (Horos 3.0). Results The statistical analysis showed a significant difference between the hypodivergent and hyperdivergent group regarding buccal bone height (P = 0.005), buccal apical bone thickness (P = 0.003) and palatal mid-root bone thickness (P = 0.006). Moreover, buccal bone height (P = 0.006) was found to be statistically significant different in normodivergent compared with hypodivergent individuals. Conclusions Facial types were found to be correlated with alveolar bone thickness. The hyperdivergent subjects presented thinner alveolus bone in the anterior maxilla and at almost all sites in the mandible. Clinicians should be aware of the possibility of thin cortical bone plates in hyperdivergent patients, reducing antero-posterior movements to avoid fenestration and dehiscence.
