Relationship among Visual Field, Blood Flow, and Neural Structure Measurements in Glaucoma

Abstract: There is a close link between reduced retinal blood flow and visual field loss in glaucoma that is largely independent of structural loss. Further studies are needed to elucidate the causes of the vascular dysfunction and potential avenues for therapeutic intervention. Blood flow measurement may be useful as an independent assessment of glaucoma severity.

“…Sectoral blood flow has advantages over total retinal blood flow, 10,18 including, for instance, that it is possible to compare sectoral flow to sectoral nerve fiber loss or visual fields in glaucoma. 19 With measurements of oxygen saturation in individual retinal vessels, it will be possible to determine sectoral oxygen extraction, which, together with sectoral flow, will help to determine the local metabolic rate of oxygen consumption in the retina.…”

“…For this reason, we developed a semiautomated software tool, termed the Doppler OCT of Retinal Circulation (DOCTORC), to allow efficient manual identification and correction of corresponding vessel positions, and subsequent generation of retinal blood flow results. [8][9][10] With high-quality scans, the good reproducibility of this approach has been described in previous publications. [9][10] However, this method has important limitations that reduce the yield of scans with accurate or valid blood flow results.…”
