Relations of Serum Aldosterone to Cardiac Structure

Abstract: Abstract-Aldosterone is associated with myocardial fibrosis in experimental studies and with left ventricular remodeling in heart failure patients. We hypothesized that aldosterone influences ventricular remodeling in people without congestive heart failure in the community. We examined the relations between serum aldosterone and echocardiographic left ventricular measurements in 2820 Framingham Study subjects (mean age 57 years, 58% women, 88% white) free of myocardial infarction and overt heart failure. Seru… Show more

“…[24][25][26] It has been suggested that hypertensive incidents and organ damage in humans may occur even at plasma aldosterone levels within the normal physiological range. 24,[27][28][29] Therefore, targeting aldosterone synthesis and release may be clinically important in preventing cardiovascular disease. Ca 2+ ions are conveyed through the T-type Ca channel to the mitochondria, where they activate aldosterone synthesis, which in turn stimulates T-type Ca channel expression, 30,31 creating a positive feedback loop of aldosterone biosynthesis in adrenal cells.…”

“…Accordingly, it could be speculated that the presumably low levels of endogenous estradiol in our female and the presumably low levels of testosterone in our mainly elderly male study population may be partly responsible for the observed higher susceptibility of female hearts for hypertrophic stimuli, i.e., adiposity. Additional explanatory effects could be interactions between the signaling effects of estrogen and aldosterone receptors (39,40); however, in the largest study, sex-related differences of serum aldosterone on left ventricular geometry were independent of menopausal status and BMI (41).…”

“…It was reported that LVH has a greater impact on survival in women than in men both in the patients with and without coronary artery disease (16,17 (18)(19)(20)(21)(22)(23)(24). The two major factors in the pathogenesis of LVH postulated on the basis of experimental results are hemodynamic and non-hemodynamic factors (24).…”
