Relating the number of human leucocytes antigen mismatches to pregnancy complications in oocyte donation pregnancies: study protocol for a prospective multicentre cohort study (DONOR study)

Abstract: IntroductionOocyte donation (OD) enables women with reproductive failure to conceive. Compared with naturally conceived (NC) and in vitrofertilisation (IVF) pregnancies, OD pregnancies are associated with a higher risk of pregnancy complications. The allogeneic nature of the fetus in OD pregnancies possibly plays a role in the development of these complications. The objective of the current study is therefore to study the number and nature of human leucocyte antigen (HLA) mismatches between fetus and mother an… Show more

“… 42 For pregnancy check-ups and delivery, these women return to the Netherlands, which poses the opportunity of participation in our study. Currently, the DONOR cohort 31 consists of 300 OD pregnancies, of which data on fetal–maternal HLA matching is available for 150 pregnancies. To increase inclusion rate and to optimise representation of the Dutch population that applies to OD, also other centres that perform OD are approached for collaboration.…”

“…To estimate a causal relation between OD pregnancy and the development of hypertensive complications using observational studies, adjustment in the analyses is of high importance. Possible associated covariates are visualised in a directed acyclic graph previously published in the protocol for the DONOR study, 34 highlighting the confounding factors that need to be adjusted. These confounding factors include maternal age, ethnicity and plurality.…”
