Relapsing cutaneous polyarteritis nodosa associated with streptococcal infections

Abstract: Various infectious and non-infectious conditions have been linked both to the initiation and relapse of this disease. We describe a patient with recurrent episodes of cutaneous polyarteritis nodosa spanning a period of over 20 years with each episode appearing to be linked to a prior streptococcal infection.

“…Several infectious and noninfectious conditions have been associated both to initiation and relapse of the disease (2, 5, 7). Among them, streptococcal infection has been commonly implicated (6, 8). Although some evidence of streptococcal infection as an initiating factor for CPAN is present, caution must be exercised when interpreting elevations in the serologic markers of streptococcal infection in the absence of an appropriate clinical presentation.…”

“…However, the patient had no history of conditions predisposing to atherosclerosis and arterial embolism, such as hypertension, diabetes, hyperlipidaemia or cardiac arrhythmias. Streptococcal infection has been implicated in relapse of CPN, 5 but the patient had no infection before admission. We prescribed minocycline hydrochloride to reduce inflammation in our patient; there are several reports on minocycline‐induced CPN, 6 which are closely associated with the presence of myeloperoxidase ANCA and improved by discontinuation of minocycline.…”
