Reirradiation for progressive brain metastases

Abstract: Brain metastases constitute one of the most common distant metastases of cancer and are increasingly being detected with better diagnostic tools. The standard of care for solitary brain metastases with the primary disease under control is surgery followed by radiotherapy. Radiotherapy is also the primary modality for the treatment of multiple brain metastases, and improves both the quality of life and survival of patient. Unfortunately, more than half of these treated patients eventually progress leading to a … Show more

“…Considering that only a certain number of patients can benefit from an active retreatment with SRS, it is necessary an appropriate patient selection. Generally, selection is based on good KPS, absence of debilitating neurologic deficits, 1–4 BM, primary controlled and/or indolent disease, and previous WBRT carried out at least 4–6 months before reirradiation [1] , [2] , [8] , [24] , [25] . These selection criteria should be adopted not only when reirradiation of BM was done with no dedicated linear accelerators but also with dedicated machines as GammaKnife or CyberKnife [26] .…”

“…Application of QUARTZ trial in clinical practice has shown that gender, performance status and EGFR mutation had impact on survival (10). However, almost half of the patients treated with WBRT gradually develop progressive intracranial disease (11). The median survival of lung cancer patients with brain metastases is around 4 months (12).…”

“…Re-irradiation (re-RT) in such patients can be offered using WBRT, partial brain radiotherapy (RT) or high dose conformal stereotactic radiotherapy (SRT). But considering tolerance dose of brain and weighing the risk-benefit ratio of re-RT, only 3-10% of patients are offered re-RT (11).…”

“…More than half the patients who are treated for brain metastases will in their lifetime eventually present with progressive disease in the brain, and more and more often as the only site of progression 18. Patients present with multiple symptoms such as headache, motor deficit, impaired mentation and seizures, where headache being the most common presenting symptom at debut of brain metastases, while motor deficit is the most frequent symptom at recurrence 19.…”
