Reidel’s thyroiditis and Tolosa-Hunt syndrome, a rare association

Abstract: A previously undescribed association of Reidel's thyroiditis with Tolosa-Hunt syndrome is reported. A 33 year-old Caucasian female presented with a two-year history of painful progressive goitre, histologically a Reidel's thyroiditis. She underwent left partial thyroidectomy. Post-operatively she developed severe right retro-orbital pain, right temporal headache and a partial right occulomotor nerve palsy. A diagnosis of Tolosa-Hunt syndrome was made. Some important aspects of this case are discussed.

“…The bibliographical search revealed 536 articles, 48 of which met the criteria to be included (8–11, 13–56). Overall we included 62 subjects extracted from 48 reports: 38 in the THS group (three of which showed a normal MRI and three were not treated), 16 in the THS plus group and eight in the OM group.…”

“…Tamoxifen is a non-steroidal selective oestrogen-receptor modulator (SERM) of the triphenylethylene family, which include clomifene, nafoxidine, ospemifene and toremifene [ 101 , 102 ]; and is structurally derived from diethylstilbestrol-type oestrogens and antioestrogens, such as chlorotrianisene and ethamoxytriphetol. Clomiphene was synthesized initially and then tamoxifen was developed [ 103 – 105 ].…”

“…Evidence for this pathogenic mechanism comes from associations between Reidel's thyroiditis and other forms of fibroinflammatory disease [idiopathic retroperitoneal fibrosis, inflammatory pseudotumors, sclerosing cholangitis, and Tolosa-Hunt syndrome (3)(4)(5)(6)]. Additionally, Reidel's thyroiditis may have an immune component as evidenced by reports of antimicrosomal and antithyroglobulin antibodies in some patients with Reidel's thyroiditis (7).…”
