Reestablishment of Occlusal Vertical Dimension in Complete Denture Wearing in Two Stages

Abstract: The assessment and reestablishment of the occlusal vertical dimension (OVD) are considered important factors in the treatment of complete denture wearers. The long-time use of a complete denture can result in jaw displacement due to abrasion of the artificial teeth and residual ridge resorption, causing esthetic complications. Most patients with old dentures and incorrect OVD accept reestablishment of the OVD with new complete dentures, even if they were used to their old dentures. The present clinical report … Show more

“…For this reason an initial adaptation period with a provisional prosthesis at appropriate occlusal vertical height is required before final restoration. 16 This allows a prudent assessment of patient's tolerance, functioning and esthetics at the proposed occlusal vertical dimension. An increase of upto 5mm in occlusal vertical dimension is an achievable alteration, as physiologic OVD occurs at a range, known as the comfort zone, rather than a specified value.…”

“…According to Kelly, extensive maxillary and mandibular ridge resorption and alveolar bone loss can result in the "flabby ridge," a mobile piece of tissue mainly seen on rugae and the tuberosity region in the maxilla, as well as the anterior crest of the ridge and retromolar area in the mandibular denture [6]. This type of soft tissue is mostly present in denture wearers in the long run, leading to discomfort and ill-fitting dentures [7][8][9]. In today's image-conscious society, dentures restore a natural appearance, encouraging patients' confidence and facilitating easier social interactions.…”

“…If the flabby ridge is not appropriately handled therapeutically, the true issues come when constructing new dentures. The specific impression procedures of the prosthetic field constitute a viable answer for the success of the mobility treatment if the flabby ridge is kept while fabricating new dentures [7,8].…”
