Reduction of fractured nasal bones; local versus general anaesthesia

Abstract: This prospective study was designed to asses whether the results of reducing simple fractures of the nasal bones were different using local or general anaesthesia. Two consecutive groups of 50 patients had their fractures manipulated under (a) general, or (b) local anaesthesia. The results of reduction as assessed by the patients and surgeons were not significantly different between the 2 groups. The reoperation rate for nasal obstruction and external deformity was similar in the 2 groups. We suggest that frac… Show more

“…Increasing evidence shows that patients have persistent aesthetic concerns about the outcome and obstructive symptoms after closed reductions. The incidence of post-reduction nasal deformities that require rhinoplasty or septorhinoplasty ranges from 14 to 50% [10,11]. Patients dissatisWed with closed reduction constituted 29% in a study done by Hung et al [12].…”

“…The need for earlier septorhinoplasties is based on two facts: the significant incidence of postreduction nasal deformities requiring rhinoplasty or septorhinoplasty, which ranges from 14% to 50% (7)(8)(9)(10), and the majority of patients who usually elect not to undergo a second operation (11).…”

“…15 The advantages of reduction and splinting under local anesthesia over general anesthesia include (1) shortened hospital stay, (2) ability of the patient to see cosmetic appearance after manipulation while still under anesthesia allowing further adjustment, (3) minimal delay between diagnosis and treatment, (4) reduction in operative costs, (5) elimination of general anesthesia risks, (6) option of general anesthesia available if local fails. 16,17 Bone healing may occur more rapidly in the pediatric population, and so splinting for two weeks is enough. 6 Four osteotomies were done with external rhinoplasty approach, and so POP cast was appropriate for them.…”
