Reducing Maternal Depressive Symptoms Through Promotion of Parenting in Pediatric Primary Care

Abstract: We studied associations between two pediatrics primary care interventions promoting parental responsiveness and maternal depressive symptoms among low-income mothers. This RCT included two interventions (Video Interaction Project [VIP], Building Blocks [BB]) and a control group. VIP is a relationship-based intervention, using video-recordings of mother-child dyads to reinforce interactional strengths. BB communicates with parents via parenting newsletters, learning materials and questionnaires. At mean (SD) ch… Show more

“…Screening of infants and toddlers as well as parents for mental health risk and referral to services leads to improvements in mental health and parenting behavior. 44,45 The Reach Out and Read program, which provides parents with children’s books along with guidance and modeling of reading interactions during the pediatric primary care visit, has demonstrated effectiveness on child language development and parent–child interaction. 46 An innovative expansion of this program using video-based feedback to enhance parent–child interaction has been shown to increase parent stimulation for development and to reduce maternal depression.…”

“…26 Previous studies of VIP have shown beneficial effects on parent-child interactions and reduced family psychosocial stressors, including maternal depression and parenting stress. [27][28][29][30] BB is a lower intensity intervention that uses mailed newsletters highlighting positive parenting strategies. BB has also been shown to enhance responsive parenting and reduce maternal depressive symptoms, although the effects are smaller.…”

“…BB has also been shown to enhance responsive parenting and reduce maternal depressive symptoms, although the effects are smaller. 27,28 The present article describes findings from a randomized controlled trial (RCT) examining the effects of VIP and BB on parenting, child development, and school readiness. The present analyses examined effects of VIP and BB on children's socioemotional development from 14 to 36 months, a primary outcome of the RCT.…”

“…30,31,34 In a subsequent RCT, these findings have been replicated and extended, showing continued effects on parenting and cognitive stimulation, 35 including enhancements in observed parent verbal input in the context of shared book reading, 36 reduced electronic media exposure, 37 and reductions in harsh discipline, 38 as well as enhancements in child socioemotional development 39 and some reductions in referral to Early Intervention Services. Additionally, effects of VIP have also been shown on a number of psychosocial factors known to affect quality of parent–child interactions such as maternal depressive symptoms 40 and parenting stress. 41 Notably, there are current efforts in New York City through the City’s First Readers initiative 42 to integrate VIP with ROR, and link these interventions with other community-based programs with similar goals.…”
