Introduction: Campylobacter jejuni is the most common cause of human bacterial enteritis in developed countries and is a rare cause of extra-intestinal infections. Case presentation: In this paper, we report a case of septic arthritis related to C. jejuni infection in an immunocompetent 53-year-old man with prosthetic devices who presented to us with enteritis. Following treatment with ciprofloxacin, loperamide hydrochloride and acetaminophen, he developed articular pain with painful swelling and redness in the region of the outer side of the left thigh. A diagnosis of a monoarticular non-migratory manifestation was postulated and vancomycin was started but was stopped 1 day later due to the development of skin erythema, and levofloxacin, teicoplanin and rifampicin treatment was started. An X-ray of the hip excluded dislocation of the prosthesis, while ultrasound showed an abscess in the soft tissue, confirmed by a triphasic bone scan. Aspiration of the joint fluid revealed the presence of moving, curved, Gram-negative bacilli 72 h after incubation, while growth on agar plates and tryptose broth remained negative after 5 days. Reactions for Campylobacter diagnosis were positive, and the micro-organism was identified as C. jejuni. Antimicrobial susceptibility tests revealed a sensitivity for ampicillin, ciprofloxacin, imipenem, tetracycline, erythromycin and gentamicin. Antimicrobial treatment was continued for a further 10 days with complete resolution of symptoms. Conclusion: Clinicians should consider Campylobacter in infections without a history of a travel in the tropics. Antibiotic treatment must be carefully evaluated to take into account the local resistance to avoid clinical failure.

Introduction: Current atrial fibrillation (AF) guidelines recommend flecainide as a first-line rhythm control option in patients without structural heart disease. While there is proven efficacy in clinical trials and guideline support, it is hypothesized that flecainide may be underutilized due to negative outcomes in the CAST trial and that adverse effects are less common than previously perceived. Methods: This retrospective chart review evaluated patients ⩾18 years initiated on flecainide for AF from August 2011 to October 2016 by a cardiology provider at the study site. Exclusion criteria included: <5 days of flecainide therapy, AF due to a reversible cause, and inadequate documentation. The primary outcome was efficacy of flecainide at maintaining symptomatic control at 6 and 12 months. Secondary outcomes included characterization of alterations in rhythm control strategies and documented normal sinus rhythm per electrocardiogram at 6 and 12 months. Results: Of the 326 patients identified, 144 patients were included. After 6 and 12 months, 102 patients (70.8%) and 89 patients (61.8%) of the 144 were symptomatically controlled. Atenolol use ( p = 0.024), female sex ( p = 0.006), hypertension ( p = 0.040), and dronedarone failure ( p = 0.012) were associated with flecainide discontinuation at 6 months. At 12 months, only previous propafenone failure ( p = 0.032) was significant. Of the 144 patients, 16 (11.1%) reported adverse effects with dizziness, hot flashes, bradycardia, and headache (1.4% each) being the most common. Conclusion: Flecainide is a well-tolerated medication, even at 12 months, with very minor adverse effects. These results support the utility of flecainide in guideline recommended patient populations.

Objective: As population increase, Healthcare resources are stretched and the need to reduce prescription drug spending becomes obvious. The aim of this study was to evaluate the levels of generic prescribing in place of their branded counterparts among type 2 diabetic and hypertensive outpatients in Eastern region of Saudi Arabia. Method: A cross section of 13 Hospitals and 17 Community Pharmacies located in Al Ahsa, Dhahran, Khobar and Dammam (both Government and private) were visited. Out of which 58 Physicians and Pharmacists were interviewed with a structured questionnaire. Reasons for prescribing and serving either generic or branded were also ascertained. Patients' prescriptions were also examined at the various hospital and community pharmacies. Our focus was on prescription drugs for type 2 diabetic and hypertensive patients only and the study lasted for 6 months. Results: Findings revealed that only 26.9% generic medications were prescribed for both type 2 diabetic and hypertensive patients in the 13 hospitals sampled. It was also observed that while Government hospital prescribed 33.3% generics, private hospitals had 28.6% for same patient category. In the Community Pharmacies, only 23.5% generic were served for diabetics and 11.8% for hypertensive patients. Overall Hospitals branded prescription were 73.1% whereas in community Pharmacies, they represented 82.4% served medications. Conclusion: Levels of generic medication usage appeared to be significantly low. Majority of the Physicians and Pharmacists were of the belief that branded medications give better outcome to patients and this attitude could potentially render Government policies on the use of generic medication ineffective.
