Recovery and Rehabilitation after Intracerebral Hemorrhage

Abstract: About half of patients survive intracerebral hemorrhage (ICH), but most are left with significant disability. Rehabilitation after ICH is the mainstay of treatment to reduce impairment, improve independence in activities, and return patients to meaningful participation in the community. The authors discuss the neuroplastic mechanisms underlying recovery in ICH, preclinical and clinical interventional studies to augment recovery, and the rehabilitative and medical management of post-ICH patients.

“…A decreased incidence of mortality, morbidity and dependency have been reported in stroke patients who undergo therapeutic training in an inpatient unit than those who receive general rehabilitation in a nondedicated unit [71]. Medically stable neurological patients need to be placed in the best possible unit for which several deliberations need to be considered [11]. On termination of a course of inpatient rehabilitation, further therapy may be instituted on an outpatient basis or an extended functional training at home may be considered [11].…”

“…The main neurological sequellae after ICH are similar to those seen after ischemic stroke: motor and sensory deficits, aphasia, neglect, cognitive dysfunction or depression [13]. After completing rehabilitation program, different results were obtained in the recovery of upper and lower limb function, sensory function or trunk control.…”

“…In inpatient facilities, patients receive intense programs, 3 hours of daily therapy 5 to 7 days per week, but for shorter periods (usually less than 1 month). In outpatient facilities or in home-based recovery, the duration is 0.5-1 hour per day of therapy, 2-3 times per week, but for 1 to 2 months [13].…”
