Recent developments in predicting thyroid malignancy

Abstract: Fine-needle aspiration biopsy of large thyroid nodules has a high false-negative rate and should be considered for diagnostic lobectomy. Nodule size appears to be an independent factor predicting malignancy and indeterminate lesions at least 4 cm should be considered for initial total thyroidectomy. Nuclear imaging may aid exclusion of malignancy in thyroid nodules and molecular markers have great promise in predicting thyroid malignancy with higher specificity.

“…A nodule size > 3-4 cm seems to be associated with a higher chance of malignancy in these cases (7), but this has not been demonstrated in all series (3,8). In the case of nodules ≥ 4 cm, our experience showed a 35% frequency of malignancy (unpublished data), a rate slightly higher than that observed in the present study for nodules < 4 cm (24%).…”

“…When cytology was indeterminate (follicular neoplasm), histology revealed malignancy in 35% of cases (versus 23.5% in nodules < 4 cm, with the same cytological result and systematically operated at Santa Casa de Belo Horizonte during the period of this studyunpublished data). Nodule size has been confirmed as a predictor of malignancy in cases with indeterminate cytology in most series, but not in all (11). Most (77%) carcinomas in nodules with indeterminate cytology (follicular neoplasm) were papillary and not follicular carcinomas.…”

“…2 Thyroid nodules can be palpated in 4% to 7% of adults. 3 However, they are found incidentally in up to 40% of patients who undergo ultrasonography of the neck. 4 Some studies estimate that 20% to 76% of the population has at least one thyroid nodule.…”
