Reasons for failed endovascular recanalization attempts in stroke patients

Abstract: PurposeMechanical thrombectomy (MT) is a highly effective therapy in patients with acute ischemic stroke due to large vessel occlusion (LVO). However, complete recanalization of the occluded vessel cannot be achieved in all patients, leading to poor clinical outcome. We analyzed the reasons for failed recanalization to help direct future improvements in therapy.Methods648 consecutive stroke patients with LVO and an MT attempt were retrospectively analyzed for none or minimal recanalization, assessed according … Show more

“…In 20% of the failing interventions, the thrombus could not be reached or passed with the device while in 80% recanalization failed due to other reasons. Literature reports similar data [13]. For patients failing due to mechanical reasons, direct carotid puncture or surgical vascular access, which is currently not performed at our center, could be a therapeutic option in certain cases.…”

“…Reasons for reperfusion failure have been investigated in single center studies on a procedural level (e.g. analyzing anatomical and technical difficulties) and have been classified into technical obstacles in reaching the occlusion site vs. failure to remove the clot [3,4]. Furthermore, EVT failure has been found to be associated with advanced patient age and distal occlusions [4].…”

“…Despite the proven efficacy of MT, treatment fails in a small subset of patients (approximately 10%) 3. Leischner and coauthors examined this cohort, to determine the technical and anatomic factors that precluded successful recanalization.…”
