The purpose of this study was to investigate the histopathological and immunohistochemical characteristics of so-called "denture fibroma" and related stromal reaction. The tissues were histologically classified into inflammatory granulation type, matured fibrous type and myxoid type. Tenascin, LOX, fibrinogen and COX were strongly detected in inflammatory granulation type whereas a weak expression was detected in matured fibrous type. LOX was remarkably expressed by spindle shaped cells/fibroblastic cells and endothelial cells while tenascin was relatively expressed by monocytes in myxoid type. Despite strong expressions of tenascin, fibrinogen, COX-2 and LOX, their expressions decreased towards the deep part of the connective tissue. The greatest number of vessels was obtained in inflammatory granulation type and the least number of vessels was obtained in myxoid type which might contribute to hypoxic condition.The results suggest that the so-called "denture fibroma" is a consequence of acute inflammatory reaction leading to the formation of inflammatory granulation followed by the state of fibrosis which gradually was replaced by matured collagen fibers. Furthermore, myxoid degeneration occurred when blood vessels decreased which lead to hypoxic condition.

Il affecte préférentiel-lement les femmes lors de la 2 e et la 3 e décades et touche surtout la région antérieure. Il est caractérisé par un taux de récidive relativement élevé. Un cas de fibrome cémento-ossifiant périphérique maxillaire postérieur chez une femme de 60 ans est rapporté. Les aspects clinique, histologique, thérapeutique ainsi que les différentes controverses dans la littérature concernant la nomenclature et l'étiopathogénie sont discutés. The peripheral cemento-ossifying fibroma (PCOF) is a non-neoplasic reactive enlargement of the gingiva characterized by the formation of mineralized product in a fibroblastic stroma. The large number of names used for PCOF indicates that there is much controversy surrounding the classification of this lesion. Although the etiology of PCOF remains unknown, it is thought to arise from periodontal ligament. It predominantly affects females at the 2nd and 3rd decade and occurs often in the anterior area. The recurrence rate of the PCOF is considered high for reactive lesions. The present report describes a case of PCOF in a 60 year-old female, localized in the posterior maxilla. Clinical and histological aspects, in addition to differential diagnosis and treatment are discussed.En raison des nombreux facteurs irritants présents dans la cavité buccale (biofilm, tartre, prothèses, etc.), la gencive peut être le siège de lésions réactionnelles très diverses. Parmi ces proliférations gingivales non néoplasiques, le fibrome cémento-ossifiant périphérique (FCOP) présente la particularité d'associer différents tissus minéralisés (os, cément, calcifications dystrophiques) dans un stroma fibreux. Dans la littérature, plusieurs termes ont été employés pour désigner le FCOP témoignant de l'ambiguïté qui entoure la nature exacte de cette lésion.Les auteurs rapportent un cas rare de localisation maxillaire postérieure d'un FCOP en discutant les aspects clinique et histologique spécifiques de cette lésion gingivale ainsi que le diagnostic différentiel et l'attitude thérapeutique.

An alternative method for esthetic and functional management of oral pyogenic granuloma

BackgroundPyogenic granulomas (PGs) are gingival enlargements which cause esthetic and functional problems that hinder chewing and maintaining oral hygiene. In this series consisting of six cases, we report the rehabilitation of PG using partly de‐epithelialized gingival grafts.MethodsFollowing documentation of clinical measurements, a concurrent treatment plan of excision and reconstruction with partly de‐epithelialized gingival grafts were followed for all the cases. Six months following procedures, clinical parameters were measured again and a short patient‐reported outcome measures consisting of three questions were applied.ResultsIn histological evaluations, PG features were observed. In the fourth postoperative week, interdental papilla and attached gingiva were restored. Six months follow‐up showed reduction in plaque and gingival indices, clinical attachment loss, and mobility. Mean keratinized tissue height was increased from 2.58 ± 2.20 to 6.66 ± 1.66 in the 6th month of operation. Oldest case was stable after 12 months of follow‐up and no infection was found at grafting sites. Papillary coverage was achieved.ConclusionIf the PG is not completely removed due to esthetic reservations, a recurrence may occur. Within our limitations, we can suggest that the immediate esthetic rehabilitation with partly de‐epithelialized gingival graft presents a compatible approach in the treatment of mucogingival defects after aggressive excision of PG.
