Background: Disc derangement disorders of the TMJ are a group of intra-articular biomechanical disorders in which there is an abnormal relationship in the functional 'articular cartilaginous' condyle-disc complex. Platelet-rich plasma (PRP) is a blood product that allows in a simple, low cost, and minimally invasive way to obtain a concentration of many growth factors when activated. Aim: To compare the use of arthrocentesis alone and arthrocentesis in addition with platelet rich plasma in management of TMJ internal derangement. Materials and Methods: fourteen patients with DDwOR were included in the study diagnosed according to The Research Diagnostic Criteria for Temporomandibular disorders (RDC/TMD) and divided into two groups as follow; seven patients underwent intra-articular injections of 2 ml PRP after arthrocentesis (study group) and the other seven patients were treated by the conventional arthrocentesis using Ringer's solution (control group). The patients were clinically evaluated preoperatively and postoperatively at the intervals of 1 month and 6 months. Results: The differences between the two groups in all the measured parameters were statistically insignificant throughout the postoperative period. Conclusion: injection is a safe and effective method in the treatment of TMDs.

A b s t r a c tThe aim of the study is the evaluation of the clinical outcome and effectiveness of the arthroscopic procedures of the temporomandibular joint (TMJ) in cases resistant to the conservative therapy. The monitored group consisted of 31 patients, who underwent the arthroscopy of the TMJ and have been followed up for 3 years. The followed up patients have underwent a MRI examination of TMJ. The authors evaluate the effectiveness of the treatment in relationship to the clinical picture, radiodiagnostical imaging methods and to the arthroscopy of the TMJ.

The symptoms of TMJ Osteoarthrosis/Osteoarthritis(TMJ OA) are characteristic and in most cases specific to the disorder. There is, however, an overlap of symptoms with other TMDs. Treatment of TMJ OA is generally directed towards management of the symptoms as the temporomandibular joint does retain the capacity throughout life to repair and remodel. Non-conservative and invasive treatment is only rarely necessary. It should be remembered that the mandibular condyle retains the capacity to repair and remodel throughout life and patients should be encouraged to bear this in mind. Conservative measures are usually sufficient to treat symptoms and a period of time should be allowed to elapse before undertaking a more aggressive treatment plan, which, in the overwhelming majority of patients, will not be necessary. This disorder should be approached on the terms of ‘a quality of life issue’ and treatment should be directed towards patient needs.
