We investigated all-inside ankle arthroscopy for Tillaux-Chaput fractures. We retrospectively evaluated 19 patients (12 males [63.2%] and 7 [36.8%] females; 11 right [57.9%] and 8 left [42.1%] ankles) treated from May 2013 to January 2016. Their mean age was 28.1 (range 10 to 55) years. Sixteen (84.2%) had single Tillaux-Chaput fractures and 3 (15.8%) had combined proximal fibular fractures; 17 (89.5%) were diagnosed radiographically and 2 (10.5%) by computed tomography. Anterolateral and anteromedial ankle arthroscopy was used for closed reduction and internal fixation with 1 or 2 Herbert screws. Joint function was evaluated using the American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot scale. All 19 patients healed by first intention without nerve, vessel, or tendon injuries. The follow-up was 19.0 (range 12 to 25) months; fracture union was achieved by 23.5 (range 12 to 36) weeks. At the last follow-up, the 19 patients had no restrictions in ankle function and range of motion, with no ankle or walking pain. The AOFAS score changed from 52.8 to 91.7 points, with an excellent/good rate of 100%. These results suggest that anterolateral and anteromedial all-inside ankle arthroscopy is a precise and effective method for closed reduction and internal fixation of Tillaux-Chaput fractures.

This case study discusses the treatment of distal methapyphiseal fractures of shin in children. The choice of this problem based on wide expansion of fractures and poor treatment results. Age of children ranges from 10 to 16 years. Examination was done with X-rays, computed tomography (CT), and ultrasound, especially in cases where damaged ligamentous apparatus was suspicious. The main method of treatment was surgicalosteosynthesis by pins, plates, and screws. In the rehabilitation period, the legs were immobilized by Plaster of Paris for 46 weeks. The first case demonstrated the probability of damage of the distal tibiofibularis ligaments and necessity of their reconstruction. In the management of the second case, the efficacy of CT scans in validation of the fracture is presented. The treatment of the teenager presented in the third case was based on biomechanical principles. In majority of cases, outcomes in the period of 68 weeks after trauma were good and satisfactory. In our opinion, diagnosis and treatment of distal methapyphiseal fractures of the shin in children must be different that was proved in the examination of bones and ligaments injuries. Creation of the special structure classification for understanding and effective treatment of these kinds of methepyphiseal fractures in children is believed to be necessar.

Ankle syndesmosis injuries occur in 1% of pediatric ankle trauma. In the younger populations, an open physis has been thought to be a protective factor against syndesmotic injury and therefore favors more Salter-Harris type injuries. Clinical methods used to diagnose syndesmotic injuries may not be effective in the pediatric population. Radiographic findings are mostly based on the study of adolescent and adult populations. Therefore, syndesmotic injuries may be overlooked in the younger pediatric patient with ankle trauma and open physes. Here we present a review of the literature of ankle syndesmotic injuries and demonstrate how isolated injury can be missed in the younger pediatric patient. Further research is needed to fully characterize clinical exam and radiographic findings of syndesmotic injury in the younger pediatric patient and how these may change with time and growth. Key Concepts• Diagnosis of ankle syndesmotic injuries, including isolated injuries, in children is difficult and has different criteria for diagnosis than in adults.• Syndesmotic anatomy changes with growth and false positives or negatives in diagnosis are possible using plain radiographs alone.• Stress radiographs, bilateral radiographs, and MRI imaging should be considered when clinical suspicion is high.• Consider tightrope fixation in lieu of syndesmotic screws as recent literature in adolescents supports it as equivalent or superior regarding patient outcomes.
