Rapid growing cystic variant of choroid plexus papilloma in a fetal cerebral hemisphere

Abstract: Rapid growing cystic variant of choroid plexus papilloma in a fetal cerebral hemisphereWe report the case of a congenital cystic choroid plexus papilloma (CPP) located in the fetal left cerebral hemisphere, in which there was a marked increase in the size of the cyst 1 week before delivery. A 26-yearold Japanese woman was referred to our hospital at 33 weeks of gestation for evaluation of a fetal brain lesion. Ultrasound examination showed a 3.0 × 4.5-cm cystic tumor involving the posterior horn of the left la… Show more

“…Minor calcification of the fibrovascular stroma is not an uncommon sign. Expansion of cysts accompanying a CPP may be seen,27 and deterioration of cerebral tissue through cystic development is found sporadically 28. In our case we saw a relatively high mitotic activity, leading to a WHO Grade II classification.…”

“…On T2‐weighted imaging, the lesion is hyperintense 1, 32. CT shows an isodense to hyperdense lesion often with intra‐tumoral calcifications and with uniform contrast enhancement 6, 28, 30. On transabdominal fetal ultrasound, CPP is an echogenic, lobulated intraventricular tumor, typically associated with feeding vessel(s) on color flow Doppler mapping.…”

“…Choroid plexus papillomas can present with cysts in 20% of cases [7,11,22]. Similar to the illustrated case, they can appear either as pure cystic tumors or as cystic tumors with solitary or multiple mural nodules [28][29][30][31][32]. Cyst formation was postulated to either be from CSF production of the tumor or hemorrhages in the tumor [7,11] although there was only one radiographically documented case of a choroid plexus papilloma with hemorrhage [10].…”

“…Cysts develop within the tumor or adjacent cortex in 7.5% of patients with CPPs. They are found on imaging either before birth, after birth, or following surgical resection [2,19,31,41,52,76]. Cysts range in size and occur on or directly beneath the ependymal lining.…”

“…Microscopically, the cysts are lined by cuboidal to columnar choroid plexus-like cells and contain pale yellow fluid. Cysts markedly increase in size in utero [52]. Most occur concomitantly in patients with CPPs, 4/53 or 7.5%.…”
