Rapid Clearance of Human Papillomavirus and Implications for Clinical Focus on Persistent Infections

Abstract: Health professionals and the public need to understand the natural history of human papillomavirus (HPV) infections of the cervix to best use the information provided by new molecular screening tests. We investigated outcomes of 800 carcinogenic HPV infections detected in 599 women at enrollment into a population-based cohort (Guanacaste, Costa Rica). For individual infections, we calculated cumulative proportions of three outcomes (viral clearance, persistence without cervical intraepithelial neoplasia grade … Show more

“…Although our virology results indicated that GTL001 þ imiquimod might enhance HPV clearance, the results may be due to or masked by spontaneous clearance, which can occur within 1 year in as many as half of HPV16-and HPV18-infected women (37). Furthermore, this phase I trial was not designed to detect significant differences in viral clearance, and we cannot exclude the possibility of transient HPV carriage following a recent sexual exposure.…”

“…Because of maximal exposure but minimal acquired immunity, HPV prevalence is high in young women (Bosch et al, 2008;McKenna and McMenamin, 2014). However, most young women have an effective immune response to clear the acute infection within a short time (Rodriguez et al, 2008). Considering the high spontaneous regression rate among this age group, HPV testing is not recommended (Davey et al, 2014).…”
