Raoultella ornithinolytica: An unusual pathogen for prosthetic joint infection

Abstract: We herein report the first case of a prosthetic joint infection caused by Raoultella ornithinolytica in an immunocompetent patient. The clinical outcome was favorable after a two-stage prosthetic exchange and a six-month course of antimicrobial therapy.

“…Furthermore, the majority of cases responded adequately to treatment, the latter consisting usually of debridement and lavage of the affected joint and subsequent prolonged antimicrobial therapy. 69 Although response to standard treatment was frequently reported as satisfactory, amputation was previously required to control the infection. 69 While some cases may suggest that a prior intervention may have led to infection, other cases were described in immunocompetent patients with no identifiable entry points.…”

Nonetheless, osteoarticular manifestations account for a minority of infectious events with Raoultella ornithinolytica in the literature.…”

“…12 De R. ornithinolytica se destaca su capacidad para adherirse a las superficies tisulares y formar biofilms, lo que explica las infecciones relacionadas con el uso de catéteres urinarios, venosos y dispositivos ortopédicos. 7 En nuestro caso, los factores de riesgo para el desarrollo de sepsis son variados: portador del catéter venoso central permanente, alteración de la microbiota intestinal y translocación bacteriana secundaria a patología de base.…”

“…These patients were considered to be at a great risk for this particular infection. Septic arthritis and sepsis in general [6,7] is possibly a major clinical consequence of this bacterial infection [8,9]. Children, especially in immunocompromised conditions are vulnerable in manifesting clinically significant complicated infections [10].…”
