“…24 We have demonstrated that there is no significant difference between responders and non-responders with regard to serum pepsinogen A, which significantly correlates with peak acid output, 8 whereas the pepsinogen A ⁄ C ratio in responders is significantly higher than that in non-responders. This ratio negatively correlates with the early morning gastric pH, 9 which may be related to basal acid output 6 . The response to antisecretory agents in patients with functional dyspepsia may depend on basal rather than peak acid output.…”

“…5 4 Moreover, Farup et al reported that it was not possible to determine H2RA responders on the basis of demographics, symptoms and signs in an investigation of 226 patients with functional dyspepsia. 6 Therefore, it is of clinical importance to categorize the population for whom H2RA provides benefit.…”

“…Several randomized controlled trials on the use of H 2 -receptor antagonists in FD have been published, but their efficacy is uncertain. Experience from everyday practice and clinical trials indicates that H 2 -receptor antagonists might be effective for a subset of patients with FD [29,30,31,32]. Therefore, it is of major clinical value to determine the characteristics of responders and nonresponders for any given treatment.…”

“…Considering long-term use of PPI therapy in FD patients, the risk of minor side effects such as vitamin B 12 deficiency, Clostridium difficile -associated diarrhea and hip fracture have to be carefully assessed during treatment. Furthermore, the management of FD remains unsatisfactory except for a selected subgroup of patients [28,29]. Several randomized controlled trials on the use of H 2 -receptor antagonists in FD have been published, but their efficacy is uncertain.…”
