Serum pepsinogen can predict response to H2‐receptor antagonist in patients with functional dyspepsia

Abstract: SUMMARYBackground: Therapy for the relief of symptoms of functional dyspepsia is unpredictable. Aim: To identify which patients may benefit from antisecretory therapy. Methods: Twenty-seven patients with functional dyspepsia were selected to receive H2-receptor antagonist (H2RA) treatment for 4 weeks. Serum pepsinogen A, pepsinogen C and gastrin were measured, and Helicobacter pylori status was determined. Symptoms were assessed at baseline and after H2RA treatment. Results: Fourteen patients were identified a… Show more

“…The results of our study suggest that measurement of gastrin and pepsinogens does not help differentiate between the various forms of GERD. However, measurement of serum pepsinogens was suggested as being helpful in identifying patients with dyspepsia for whom acid control provides benefit 34 . Yoshikawa found that dyspeptic patients that responded to H‐2 blocker therapy had a higher pepsinogen I/II ratio than nonresponders (77% vs. 13%) 34 …”

“…Functional dyspepsia overlapped with GERD involves the matter of its epidemiology and treatment. There are some reports dealing with the efficacy of PPI or H 2 RA for the treatment of FD 24, 25 . Although the acid suppressants empirically improve the symptoms of FD overlapped with GERD, there is no definite consensus for the treatment of these patients 26 .…”

“…Use of acid suppressive therapy, often applied in FD as first-line therapy, may increase gastrin levels and it remains unclear to which extent the studies could rigorously exclude such confounder. In a relatively small study from Japan, gastrin serum level did not predict the response to H2 blocker therapy in FD (33).…”
