Radiologic findings of osteoarticular infection in paracoccidioidomycosis

Abstract: All encountered bone lesions were osteolytic. Metaphyseal or meta-epiphyseal osteomyelitis of a long bone was the most prevalent osteoarticular manifestation of paracoccidioidomycosis. PCM osteoarticular involvement could be solitary or multifocal, occurs almost exclusively in the acute/subacute clinical form, and it is more common in children and in juvenile patients. Axial skeleton involvement, arthritis, or a disseminated osseous pattern of infection may occasionally occur in this fungal disease.

“…Other radiological findings, described in 19 cases of osteoarticular PCM as osteolytic lesions and marginal sclerosis, as described by MONSIGNORE et al. 25 , bone flattening 15 irregularities of the trabecular bone 23 , joint swelling and the destruction of articular cartilage, which were indistinguishable from other infectious processes 10 , were also present among our findings.…”

“…MONSIGNORE et al 25 studied the radiological findings of 19 cases of osteoarticular PCM and reported that, in all cases, osteolytic lesions were found. Other studies have also reported that lytic bone lesions are a characteristic of radiological involvement 9,11 with or without marginal sclerosis 7,13 .…”

“…In the disseminated form of paracoccidioidomycosis, osteoarticular involvement could reach up to 20% of cases [8], and some reports have found only bone involvement in the acute clinical form [16, 17]. These severe forms of this disease may be complicated to hypercalcemia and hemophagocytic syndromes [18].…”
