Radioimmunoguided surgery for colorectal cancer

Abstract: These results suggest that RIGS identifies patterns of disease dissemination different from those identified by traditional staging techniques. Removal of additional RIGS-positive tissues in nontraditional areas may improve survival.

“…Today, radio-guided methods remain a viable clinical tool for both resection (for example, radioimmuno-guided surgery in colorectal 197 and brain cancer 198 ) and biopsy (for example, 99m Tc colloids for SLN biopsy in breast cancer) 196,198,199 . An underlying strength of radionuclide imaging is that pre-surgical imaging and radio-guided surgery can be conducted with the same probes.…”

“…Fifty-five percent of patients with primary disease had additional information generated by the RIGS system-information that was not recognized traditionally-resulting in changes in surgical management in 68 cases. Changes included extending margins of resection (8), resecting additional tissues (21), biopsy of additional tissues (26), and abandoning planned resections (5). RIGS+ tissue that had not been removed because of its anatomical location or extent-or because multiple negative frozen sections removed at surgery were found to be negative by the pathologist-was identified and recorded.…”

“…In particular, the second-generation TAG-72 MAbs appear to localize to lymph nodes at a higher rate than occurs with B72.3. Many of these nodes are located in the gastrohepatic ligament, along the hepatic artery or posterior to the pancreas, as well as in the celiac axis, periaortic, portal vein, or pericaval areas [7][8][9][10][11][12][13][14][15]. Although RIGS+ , a large percentage of these nodes fail to demonstrate tumor cells on routine histology.…”

“…Progress in the field of radioimmunoguided surgery (RIGS) has been rapid and considerable in the past few years, as supported by the large number of clinical experiences reported in literature [1][2][3][4][5][6]. Despite the encouraging results reported, RIGS is still a complex and sophisticated diagnostic procedure whose clinical utility can be challenged by many factors.…”
