A BSTRACT Context: The evaluation of relationship between the roots of impacted lower third molar (IL3M) and inferior alveolar nerve injury (IAN) with orthopantamogram (OPG) is mandatory before performing de-impaction surgery. An investigation is considered reliable if it can be reproduced by various examiners. Assessment of OPG is subjective and varies among examiners. OPG is reliable to clinicians if the interpretation is not a product of guess work. Aim: The aim of this study was to evaluate the magnitude of agreement among oral surgeons and oral radiologist in observing intimate relationship between IL3M and mandibular canal. Materials and Methods: OPGs were evaluated by two oral surgeons and one oral radiologist for nerve root relationship. All the three were from different institutions with 10–15 years of experience. The three observers were blinded from each other’s findings. A total of 127 OPGs were evaluated for inter-observer agreement. Fifty OPGs were evaluated after 60 days for intra-observer agreement. The agreement was evaluated based on Cohen’s κ statistics. Results: Our results denote that the interpretation of OPG among specialists is not in good agreement. We suggest development of methods to standardize evaluation of OPG and the exposure technique to improve inter-observer agreement among the dental specialists.

The Effects of Intravenous Antibiotic Administration during the Surgical Removal of Third Molar: Systematic Review

Intravenous antibiotics a subset of parenteral route help in reducing the post-operative infections in dento-alveolar surgery such as the removal of third molar. However, this route lacks evidential support in the literature and thus has received less attention among the oral clinicians and proportionally in pharmacologists. A systematic review was performed in the Pubmed, Pubmed Central and Scopus electronic data bases from January 2003-January 2017 with a view to express the last fifteen years performance. From the primary queries, only 74 references could be collected pertinent to antibiotic therapy in third molar surgery including the protocols laid by American Association of Oral maxillofacial Surgeons and American Academic of Pediatric Dentistry. In the final stage, a couple of articles on in vitro studies, three short discussions, one evidence-based report and a letter to editor met the required desired criteria. The results revealed that it is highly preferable to administrate intravenous antibiotics in high-risk individuals to reduce surgical site infections. From the retrieved evidences, it can be asserted that candidates with clinical signs of rapid progressive infection, total osseous impaction and compromised health deserve to receive a pre-operative single dose of intravenous antibiotics during third molar surgery on the grounds of their vulnerability to bacteremia.

ÖZ GİRİŞ ve AMAÇ: Bu çalışmanın amacı, 50 yaş ve üstü bireylerde panoramik radyograflarda üçüncü molar diş prevelansının belirlenmesi ve bu dişlerle ilişkili patolojilerin değerlendirilmesidir. YÖNTEM ve GEREÇLER: Bu çalışmada, 50 yaş ve üzeri 300 hastaya ait panoramik radyograflar değerlendirildi. Üçüncü molar diş prevelansı, gömüklük durumu ve pozisyonu, bu dişlerle ilgili durumlar ve patolojiler incelendi. BULGULAR: Çalışmada değerlendirilen 300 hastada toplam 272 adet üçüncü molar diş olup; bunların 115 tanesi üst çenede, 157 tanesi ise alt çenede izlenmiştir. Üçüncü molar dişlerin %13.2'si gömülü olarak belirlenmiştir. 272 adet dişin %3.3'ünün hiçbir patoloji izlenmeyen sağlıklı diş olduğu gözlendi. TARTIŞMA ve SONUÇ: İleri yaşlarda da ağızda üçüncü molar diş görülme olasılığının fazla olduğu; kist/ tümör gibi ciddi patolojiler nadir olarak görülse de, çürük ile ilgili durumlar ve periodontal patolojilere sık rastlandığı görülmüştür.İleri yaşlarda gömülü ÜMD'lere erkeklerde kadınlara göre daha fazla rastlandığı tespit edilmiştir. İleri yaşlarda gömülü üçüncü molar dişlerin en sık vertikal pozisyonda olduğu tespit edilmiştir.Anahtar Kelimeler: panoramik radyografi, patoloji, üçüncü molar ABSTRACT INTRODUCTION: The aim of this study was to determine the prevalence of third molar teeth on panoramic radiographs in adults aged 50 and older individuals and to evaluate the pathologies associated with these teeth. METHODS: In this study, 300 panoramic radiographs of 50 years and older patients were evaluated. Third molar tooth prevalence, impacted teeth and position, conditions and pathologies related to these teeth were examined. RESULTS: There were 272 third molar teeth in 300 patients evaluated in the study; 115 of them were seen in the maxilla and 157 of them in the mandibula. 13.2% of 272 third molars were seen as impacted teeth. 3.3% of 272 teeth were observed to be healthy teeth without any pathology. DISCUSSION AND CONCLUSION: In middle aged and older individuals, the possibility of third molar tooth appearance is high in the mouth; severe pathologies such as cysts / tumors are seen rarely, but cariesrelated conditions and periodontal pathologies are common. It was found that the impacted third molar teeth in middle aged and older individuals were more common in males than females. The most common position of the impacted third molar teeth in middle-aged and older individuals was vertical.
