Radioactive Iodine Administered for Thyroid Remnant Ablation Following Recombinant Human Thyroid Stimulating Hormone Preparation Also Has an Important Adjuvant Therapy Function

Abstract: Preparation with either rhTSH or THW in this retrospective study appears to have similar therapeutic (tumoricidal) effects on small volume RAI-avid metastatic disease incidentally discovered at the time of ablation in both locoregional lymph nodes and pulmonary parenchyma.

“…Our findings confirm those of previous retrospective studies [4,20,39,40], showing the rates of ablation success and disease outcome associated with rhTSH therapy to be statistically similar or even superior to the rates achieved with THW before ablation in DTC patients who had initially had an intermediate or high risk of recurrence.…”

“…The three goals of post-surgical RAI therapy in patients with differentiated DTC are enumerated by Tuttle et al (2010): i) to ablate residual normal thyroid tissue that may facilitate surveillance. ii) As 'adjuvant therapy' because of the potential tumoricidal effect on residual microscopic RAI-avid disease.…”

“…rhTSH-assisted RAI ablative therapy is associated with similar rates of persistent disease and clinical recurrences than those observed after traditional THW preparation, at least in the short-term follow-up [6,7]. In addition, preparation with either rhTSH or THW appears to have similar adjuvant effects on small-volume RAI-avid disease identified outside the thyroid bed at the time of initial RAI ablation [4,8]. …”
