Radio-Frequency Energy Delivery to the Anal Canal for the Treatment of Fecal Incontinence

Abstract: Radio-frequency energy delivery to the anal canal for treatment of fecal incontinence is a new modality that, in this study group, safely improved Wexner and fecal incontinence-related quality of life scores, eliminated protective pad use in most patients, and improved patient quality of life.

“…Unlike RF tissue ablation, a technique used to necrose and/or shrink tissue in the treatment of disorders such as menorrhagia and benign prostatic hyperplasia, respectively, RF micro-remodeling utilizes lower temperatures to denature collagen in microscopic sites. RF micro-remodeling results in a change in luminal function (dynamic compliance) [Takahashi et al, 2002;Lufti et al, 2004;Portale et al, 2004] but does not produce gross anatomic narrowing or thickening. Extensive animal studies have demonstrated the safety and feasibility of non-surgical, transurethral, palpation-based RF micro-remodeling of lower urinary tract submucosa (unpublished data).…”

“…In our group of incontinent patients, only 12 (18%) ultimately underwent sphincter repair. Two patients were later referred for sacral neuromodulation elsewhere and eleven patients [22] were treated with SECCA ® (radio frequent energy application to the external sphincter [23,24] ). Women who experienced a 3 rd degree sphincter rupture are indicated for AFE, even without complaints.…”

“…Patients are discharged in accordance with the criteria required for conscious sedation in an ambulatory setting or general anesthesia if so performed. 5 performed the initial clinical trial in which the feasibility, safety, and efficacy of the concept of RF energy delivery to the anal canal for the treatment of fecal incontinence was investigated. Ten women, with incontinence of varying etiologies, were treated with this device.…”
