Quantitative anatomical and morphological classification of the iliac vessels anterior to the lumbosacral vertebrae

Abstract: Object. Anterior surgical approaches to the lumbosacral disc spaces are being undertaken with increasing frequency. This increase and the use of minimally invasive techniques themselves have the potential to raise the incidence of major vessel injuries. The purpose of this study was to determine the variability of the vascular anatomy anterior to the lumbosacral spine and to draw conclusions regarding surgical accessibility of the L5—S1 disc space. “…Preoperative evaluation: In addition to routine preoperative studies to determine the indications for L5-S1 fusion, midsag-ittal T1- and T2-weighted MRI images of the sacrum and coccyx were performed to rule out a vascular anomaly or tumor. 25 , 26 , 35 Evaluation of the sagittal curve of the sacrum is necessary to confirm the feasibility of the approach ( Fig. 1 ).…”

“…6 Anterior surgical approaches to the lumbosacral disc spaces even with minimally invasive techniques can cause major vessel injuries. 9,10 The variant venous trunk in the current case was situated in front of the vertebral column and can cause disastrous bleeding due to its large size.…”

“…Cho et al evaluated, in a cadaver study, the anatomical features of iliac vessels with respect to the anterior approach to the LS spine. Only 60% of the specimens were judged to have easy accessibility to the LS spine 25 . Ebraheim et al called the retroperitoneal space anterior to the LS junction the "triangular safety zone".…”
