An die Erläuterung der Aufgabenstellung schließt sich die kurze Beschreibung einiger bekannter Methoden zur Kalksalzbestimmung im Knochen an. Das Verfahren von Krokowski der photoinetrisehen Aliswertung von Hartstrahl-und Weichstrahlaufnahmen der Wirbelsäule zur Kalksalzbestimmung bei osteoporotischen Veränderungen wird ausführlich dargestellt. Auf dieses Verfahren bauen dann die Überlegungen zur Kalksalzbestimmung durch die fernsehtechnische Subtraktion solcher Aufnahmepaare auf. Es wird ein prinzipieller Weg geschildert, anhand des Differenzbildes von Hartstrahlund Weichstrahlaufnahme den Subtraktionsgrad so einzustellen, daß er ein Maß für den Kalksalzgehalt des Knochens ist. Die Einstellung erfolgt nach der Art eines Abgleich-oder Nullverfahrens. Auf die noch verbleibenden Ausführungsschwierig-keiten dieses Verfahrens wird hingewiesen.After an explanation of the task in hand a short description is given of a few well known methods determining the calcium content in bone. The method of Krokowski of photometric evaluation of high and low kV radiographs of the spine for determining the calcium content in the case of osteoporotic changes is discussed in detail. The idea of television subtraction of such pairs of radiographs is based on this method. The principle is described to control the degree of subtraction by inspection of the difference image of the high ana low kV radiographs in such manner, that it can be used as a measure for the calcium content of bone. The setting is made in a similar manner to that balancing a bridge. The difficulties still involved in carrying out this method are discussed. Die AufgabenstellungDie Subtraktion einer Röntgenaufnahme von einer anderen dient der Verdeutlichung der Unterschiede zwischen diesen beiden Aufnahmen, eine photographische Technik wird schon 1935 beschrieben [1]. Zur Verdeutlichung kontrastmittelgefüllter Gefäße in einem Körperabschnitt subtrahiert man so von der sogenannten "Füllungsaufnahme" die "Leeraufnahme", die den gleichen Körperabschnitt ohne Kontrastmittelgabe wiedergibt. Im Differenzbild werden damit die Gefäße auch dort sichtbar, wo sie in der Füllungsaufnahme durch Überlagerung von anderen Strukturen nicht mehr zu erkennen sind. Die fernsehtechnische, also elektronische Subtraktion [2] kommt solchen Verfahren vor allem dadurch entgegen, daß ohne Zeitaufwand für einen photographischen Prozeß das Differenzbild unmittelbar zur Verfügung steht. Der Subtraktionsgrad, also die Verstärkung des zu subtrahierenden Bildes, ist dabei kontinuierlich einstellbar.Man kann nun überlegen, ob sich das einfach zu handhabende elektronische Subtraktionsgerät außer für die Gefäßdarstellung auch für andere Zwecke einsetzen läßt. So bietet sich als weitere Anwendungsmöglichkeit die Differenzbildung einer Hartstrahl-und einer Weichstrahlaufnahme an. Lateral dual-photon absorptiometry: A new technique to measure the bone mineral density at the lumbar spine

Bone mineral density (BMD) measurement of the lumbar spine by dual-photon absorptiometry (DPA) using a radioactive source and more recently an x-ray tube (DEXA) is a useful noninvasive technique to assess bone loss in vertebral osteoporosis. Because an anteroposterior (AP) projection is used, DPA measures not only the mainly trabecular bone of the vertebral body but also the cortical bone of the posterior processes, which does not contribute to the development of crushed fractures. Using a DPA apparatus equipped with a 153 Gd source coupled with an 18-detector system that provides better collection efficiency for the narrow beam of 153 Gd radiation, we have developed a technique measuring the BMD of the vertebral body of L2, L3, and L4 on a lateral projection based on previous studies using radiographic photodensitometry for lateral spine. The precision of the method-1.7% in vivo and 0.88% in vitro-is similar to that obtained with the AP projection (respectively, 1.7 and 1.0%). Lateral BMD decreased with age in 143 normal women and was best accounted for by a cubic curve (r = -0.51, p less than 0.001). The age-related bone loss between 30 and 80 years of age was much higher with the lateral (-44%) than with the AP (-22%) projection, a pattern consistent with the greater trabecular than cortical age-related bone loss. When osteoporotics (OP) were compared to age-matched normal women, the decreased in BMD was larger with the lateral (-30%) than with the AP projection (-23%), but in terms of Z scores, no difference could be found between both projections.(ABSTRACT TRUNCATED AT 250 WORDS)
