Ósaeðarlokuþrengsli (aortic stenosis) er algengasti lokusjúkdómur inn í fullorðnum á Vesturlöndum. 1 Algengi sjúkdómsins eykst með haekkandi aldri og hrjáir um 2,8% einstaklinga á aldrinum 6074 ára en um 13% einstaklinga eldri en 75 ára. 2 Aldursbundin kölkun á eðlilegri þríblöðkuloku er algengasta orsök ósaeðarlokuþrengsla í fullorðnum og greinist yfirleitt upp úr sextugu en einstaklingar með meðfaedda tvíblöðkuloku (um 12% algengi) greinast fyrr á aevinni. 1,3 Meingerð ósaeðarlokuþrengsla er margþaett og flókin en loku blöðin þykkna og stífna vegna kölkunar. Þannig þrengist ósaeðar lokan og til að viðhalda útstreymishlutfalli hjartans þykknar vinstri slegillinn, sem aftur eykur á súrefnisþörf vöðvans. 46 Al gengustu einkenni ósaeðarlokuþrengsla eru maeði, hjartaöng og yfirlið en auk þess geta hjartsláttartruflanir komið fyrir og jafn vel valdið skyndidauða. 6,7 Lyfjameðferð hefur lítil áhrif á gang sjúkdómsins 8 og þegar einkenna hjartabilunar verður vart versna lífslíkur einstaklinga hratt. 4 Hefðbundin meðferð er opin hjarta aðgerð þar sem lokunni er skipt út; ýmist fyrir ólífraena loku úr hertu kolefni eða lífraena loku úr svíni eða gollurshúsi kálfs. 9 Hér á landi eru ósaeðarlokuskipti önnur algengasta opna hjartaaðgerðin á eftir kransaeðahjáveitu. 10 Á síðustu árum hefur komið fram ný meðferð við ósaeðarlokuþrengslum sem kallast ósaeðarlokuísetn ing með þraeðingartaekni, eða TAVI (transcatheter arotic valve implantion), þar sem lífraenni loku er komið fyrir í ósaeðarlokustað Snemmkominn árangur opinna ósaeðarlokuskipta við ósaeðarloku þrengslum hjá konum á Íslandi Á G R I P Inngangur Ósaeðarlokuskipti er önnur algengasta hjartaskurðaðgerðin á Íslandi. Markmið rannsóknarinnar er að meta í fyrsta sinn á Íslandi snemmkominn árangur opinna ósaeðarlokuskipta vegna ósaeðarlokuþrengsla hjá konum. Efniviður og aðferðir Afturskyggn rannsókn á 428 sjúklingum sem gengust undir opin ósaeðarlokuskipti vegna ósaeðarlokuþrengsla á Íslandi 2002-2013. Upplýsingar fengust úr sjúkraskrám og voru skráðir fylgikvillar aðgerðar og farið var yfir hjartaómanir fyrir og eftir aðgerð. Forspárþaettir dauða innan 30 daga voru metnir með lógistískri aðhvarfsgreiningu og heildarlifun áaetluð (Kaplan-Meier). Miðgildi eftirfylgdartíma var 8,8 ár (0-16,5 ár). Niðurstöður Af 428 sjúklingum voru 151 konur (35,3%) og voru þaer að meðaltali tveimur árum eldri en karlar (72,6 ± 9,4 ára á móti 70,4 ± 9,8, p=0,020). Einkenni fyrir aðgerð voru sambaerileg milli kynja en konur höfðu marktaekt haerra EuroSCORE II fyrir aðgerð (5,2 ± 8,8 á móti 3,2 ± 4,6, p=0,002). Hámarks-þrýstingsfall yfir ósaeðarlokuna var haerra hjá konum (74,4 ± 29,3 mmHg á móti 68,0 ± 23,4 mmHg, p=0,013) en tíðni snemmkominna fylgikvilla, baeði minniháttar og alvarlegra, var sambaerileg milli kynja líkt og 30 daga dánartíðni (8,6% á móti 4,0%, p=0,076) og 5 ára lifun (80,1% á móti 83,0% fyrir karla, p=0,49). Kvenkyn reyndist ekki vera forspárþáttur fyrir dauða innan 30 daga þegar leiðrétt var fyrir öðrum þekktum forspárþáttum dauða (ÁH: 1,54, 95%-ÖB: 0,63-3,77) svo sem aldr...

Cardiovascular calcification is an independent risk factor and an established predictor of adverse cardiovascular events. Despite concomitant factors leading to atherosclerosis and heart valve disease (VHD), the latter has been identified as an independent pathological entity. Calcific aortic valve stenosis is the most common form of VDH resulting of either congenital malformations or senile “degeneration.” About 2% of the population over 65 years is affected by aortic valve stenosis which represents a major cause of morbidity and mortality in the elderly. A multifactorial, complex and active heterotopic bone-like formation process, including extracellular matrix remodeling, osteogenesis and angiogenesis, drives heart valve “degeneration” and calcification, finally causing left ventricle outflow obstruction. Surgical heart valve replacement is the current therapeutic option for those patients diagnosed with severe VHD representing more than 20% of all cardiac surgeries nowadays. Tissue Engineering of Heart Valves (TEHV) is emerging as a valuable alternative for definitive treatment of VHD and promises to overcome either the chronic oral anticoagulation or the time-dependent deterioration and reintervention of current mechanical or biological prosthesis, respectively. Among the plethora of approaches and stablished techniques for TEHV, utilization of different cell sources may confer of additional properties, desirable and not, which need to be considered before moving from the bench to the bedside. This review aims to provide a critical appraisal of current knowledge about calcific VHD and to discuss the pros and cons of the main cell sources tested in studies addressing in vitro TEHV.

The introduction of transcatheter aortic valve implantation (TAVI) have changed the management of patients affected by severe aortic stenosis. Nowadays, TAVI procedure represents the treatment of choice in patients with severe AS inoperable or at high risk. MDCT is the technique of choice in preoperative planning: it provides information regarding aortic valve and vascular access. Particular attention should be paid on bicuspid valve morphology. The choice of valve is, basically, based on confidence and experience of the operator and center.
