Rashmi TURIOR †a) , Student Member, Danu ONKAEW †b) , and Bunyarit UYYANONVARA †c) , Nonmembers SUMMARYAutomatic vessel tortuosity measures are crucial for many applications related to retinal diseases such as those due to retinopathy of prematurity (ROP), hypertension, stroke, diabetes and cardiovascular diseases. An automatic evaluation and quantification of retinal vascular tortuosity would help in the early detection of such retinopathies and other systemic diseases. In this paper, we propose a novel tortuosity index based on principal component analysis. The index is compared with three existant indices using simulated curves and real retinal images to demonstrate that it is a valid indicator of tortuosity. The proposed index satisfies all the tortuosity properties such as invariance to translation, rotation and scaling and also the modulation properties. It is capable of differentiating the tortuosity of structures that visually appear to be different in tortuosity and shapes. The proposed index can automatically classify the image as tortuous or non tortuous. For an optimal set of training parameters, the prediction accuracy is as high as 82.94% and 86.6% on 45 retinal images at segment level and image level, respectively. The test results are verified against the judgement of two expert Ophthalmologists. The proposed index is marked by its inherent simplicity and computational attractiveness, and produces the expected estimate, irrespective of the segmentation approach. Examples and experimental results demonstrate the fitness and effectiveness of the proposed technique for both simulated curves and retinal images. key words: tortuosity, retinopathy of prematurity (ROP), principal component analysis

The clinical recognition of abnormal vascular tortuosity is important in the diagnosis of many diseases. This paper presents a novel approach to the quantification of vascular tortuosity, using robust metrics based on unit speed parameterizations of three-dimensional (3D) curvature. The use of approximating polynomial spline-fitting obviates the need for arbitrary filtering of mid-line data which is necessary with other tortuosity indices. The metrics were tested using both two-dimensional and three-dimensional synthesized images that mimicked clinically significant pathologies: two of the three metrics were scale invariant, additive, and produced tortuosity values tailored to be independent of the resolution of the imaging system. Our methodology is designed to explicitly handle the challenge of noisy data, and is largely tolerant of the inaccuracies in the mid-line extraction. While all the proposed metrics are sensitive to gently curved vessels, the rms curvature of the smoothest path was more effective in recognizing abnormalities involving high-frequency coiling such as occurs in malignant tumors. We have also indicated how values from two projection images, such as acquired in biplane angiography, can be combined to give an accurate approximation of the 3D value of this metric. Our proposed methodology is well-suited to automated detection and measurement, which are a prerequisite for clinical implementation.

The aim of this study was to investigate the angiogenic response to exogenously administered basic fibroblast growth factor (FGF-2) in normal and ischemic skin, using the hairless mouse ear microcirculatory model. The hairless mouse ear is a well-established model for in vivo studies of skin microcirculation. Using this model, angiogenesis- and angiogenesis-associated changes in the microcirculation can be directly and continuously viewed and quantified in a variety of different experimental settings. To create ischemia in the mouse ear, all but one of the three to four feeding vessels nourishing the ear were ligated 3 days prior to a local subdermal injection of FGF-2 (9.3 + 1-0.5 mm/mm2) or saline into the dorsum of the ears. Angiogenesis was quantified by direct observation, at high magnification, of the injection site where increases in total vessel length (TVL) were measured repeatedly over 18 days following injection. We found a significant (P < 0.01) increase in TVL in normal and ischemic ears injected with FGF-2. Saline injection also induced a significant increase in TVL in ischemic ears. However, the angiogenic response to FGF-2 in ischemic ears was significantly stronger than saline alone in ischemic ears or saline or FGF-2 in normal ears. This response could be used clinically to accelerate angiogenesis and thus increase perfusion in ischemic tissue.
