Objetivo: Identificar os problemas de adaptação, segundo o Modelo Teórico proposto por Sister Callista Roy, em crianças que realizam tratamento de hemodiálise. Metodologia: Estudo transversal e descritivo, realizado com 16 pacientes, em um centro de referência em terapia renal pediátrica de uma cidade do Norte do Brasil. Para a coleta foi utilizado um formulário de entrevista e levantamento de dados presentes em prontuários, nos meses de janeiro a março de 2016. Resultados: Em relação aos dados de hemodiálise, o tempo de realização da terapia variou de 12 a 96 meses, sendo a média de 31,68 meses, ou seja, média de 2 anos e 8 meses. O acesso vascular predominante foi o cateter (87,5%). Foram identificados 11 problemas adaptativos nos modos relacionados ao fisiológico, autoconceito, identidade e interdependência. Conclusão: a utilização do modelo de adaptação de Roy foi apropriada, pois permitiu entender as alterações relacionadas aos modos adaptativos e visualizar de forma concreta quais os focos para as intervenções de enfermagem.

Background It is important to develop self‐management behaviours in adolescents to prevent chronic kidney disease progression. Objectives This study evaluates the effect of a self‐management program that was developed based on individual and family self‐management theory to aid adolescents with chronic kidney disease in the acquisition of competencies in the management of their disease. Design This is a randomized controlled study. Participants The study was conducted with 20 intervention and 20 control patients, who were treated at two hospitals in Ankara between December 2018 and May 2019. Measurements Self‐Management Assessment Form, Paediatric Quality of Life Inventory‐Adolescent Form and clinical parameters. A training program, specific to chronic kidney disease, was administered over three sessions in a group training setting when the patients came to the clinic for follow‐up. After the training program was completed, the patients were monitored for three months through home visits once every two weeks, totalling six visits. Results Following the application of self‐management training, a statistically significant difference was found self‐management knowledge and behaviours, quality of life and anxiety mean scores of intervention and control group (p <.05), a significant decrease in only blood urea nitrogen among the clinical parameters when compared to the control group. Conclusions In this study, a self‐management training program was developed based on Individual and Family Self‐Management Theory, aiming to have a positive effect on the self‐management behaviours of adolescents in the cognitive and psychosocial domains.

Objective: To investigate the emotional repercussions and quality of life (QOL) associated with end-stage kidney disease (ESKD) in children and adolescents undergoing hemodialysis or a kidney transplant (TX). Methods: We conducted a quantitative-qualitative study. 48 children and adolescents with ESKD were interviewed; half of them underwent hemodialysis treatment, and the other half had a kidney transplantation. Their respective 48 caregivers also participated in the study. The questionnaire involved both the Pediatric Quality of Life Inventory and a thematic story-drawing tool. An analysis of the QOL questionnaire’s results was done by comparing the sum of points between groups and the theme-based story-drawing consisted of interpreting the data contained in the material using Freudian and Lacanian theories. Results: In the QOL questionnaires, the total score was higher in the transplanted patients and in their caregivers, suggesting a perception of better QOL after kidney transplantation. In the specific aspects of the questionnaire, physical capacity was considered superior by children who underwent transplants and their caregivers. There were no differences between the groups in the emotional, social and school aspects. However, the caregivers of the patients who had a transplant perceived a significant difference in QOL in the school aspect. In the thematic story-drawings, emotional suffering in the two analyzed groups was evidenced regardless of the treatment. Conclusions: Despite the questionnaire results suggesting that transplantation does improve some aspects of QOL, there were no differences observed between kidney replacement therapies regarding the emotional repercussion of chronic kidney disease.
