Pyogenic Ventriculitis following Enteral Bacterial Translocation in a Patient with Small Bowel Obstruction

Abstract: The authors present a rare case of ventriculitis secondary to cerebro spinal fluid (CSF) colonization with Escherichia coli species in a 65-year-old woman. Passage of bacterial organisms from the lumen of the gastrointestinal tract to the bloodstream or lymphatic tissue is known as translocation. Once in the bloodstream, particular bacteria are able to cross the blood-brain barrier and migrate to CSF. Elective abdominal surgery, intestinal obstruction, colorectal cancer, ischaemic reperfusion injury and pancre… Show more

“…However, during severe sepsis and sepsis shock, human immunity is compromised and the permeability of the intestinal mucosa changes. E coli, as an opportunistic pathogen, can migrate from the gastrointestinal tract to the extraintestinal sites by bacterial translocation [28,29] , causing local or disseminated infection and even life-threatening secondary infections.…”

“…Cultures of urine, blood and ventricle-drained pus all grew Escherichia coli with the same antibiogram profiles including susceptibility to ceftriaxone in the drug sensitivity test. Pyogenic ventriculitis is rare but can be a serious complication from sepsis including urosepsis 1 2. Urgent diagnosis and neurosurgical drainage is mandatory for preventing neurological morbidity from this important central nervous system complication from any types of sepsis.…”
