Pulmonary fibrosis secondary to siderosis causing symptomatic respiratory disease: a case report

Abstract: Introduction: Pulmonary siderosis secondary to the inhalation of iron compounds is a rare condition which, despite striking radiological and histopathological features, has not traditionally been associated with symptoms or functional impairment. Although not the first of its kind, we present an unusual case of pulmonary siderosis with symptomatic respiratory disease, most likely secondary to associated fibrosis.

“…However, Funahashi et al (102) demonstrated that it was possible for welders to develop pulmonary fibrosis after exposure to only welding fumes. More recent case reports have confirmed these observations that in some specific working conditions pulmonary fibrosis may develop secondary to siderosis and cause symptomatic respiratory disease in welders (104,105). Silicon levels in the lung tissue of these welders were no different from controls, ruling out the likelihood of exposure to highly fibrogenic dust, silica.…”

“…The presence of silica or asbestos (mixed pneumoconiosis) modifies the pathologic findings according to the degree of this deposition. Presence of nodular fibrosis and/or centriacinar emphysema may be found in some cases (16,23,25,26) . The differential diagnosis of IPH also includes cases of vasculitis related to polyarteritis nodosa, Takayasu's arteritis, Behçet's syndrome, sarcoidosis, pulmonary infection, and septic emboli (4,5,7,9,11,15,17,25) .…”
