Pulmonary Disease Caused by Non-Tuberculous Mycobacteria

Abstract: Non-tuberculous mycobacteria (NTM) include more than 160 ubiquitous, environmental, acid-fast-staining bacterial species, some of which may cause disease in humans. Chronic pulmonary infection is the most common clinical manifestation. Although patients suffering from chronic lung diseases are particularly susceptible to NTM pulmonary disease, many affected patients have no apparent risk factors. Host and pathogen factors leading to NTM pulmonary disease are not well understood and preventive therapies are lac… Show more

“…These include the repeated isolation of mycobacteria from respiratory samples, compatible clinical and radiological presentation, and the exclusion of any alternative diagnosis. Nevertheless, these criteria are not generally applicable to all patients with NTM-PD in the same manner [34]. They may for example differ between patients with nodular-bronchiectatic and those with fibrocavitary disease, with the latter disease often necessitating a more aggressive treatment approach.…”

“…Different management approaches depending on the NTM species isolated, the underlying diseases, or different disease presentations (e.g. nodular-bronchiectatic vs. fibrocavitary disease) are discussed but not established [34]. There is a severe lack of knowledge about the correlation of in vitro susceptibility with in vivo efficacy of antimicrobial drugs, making DST-guided therapy difficult [37].…”

“…The latter is not specific to NTM infections, but the presence of bronchiectasis has been suggested to be typical in patients with Myobacterium avium complex infection. Generally, the clinical course of nodular-bronchiectatic disease seems less aggressive than that of fibrocavitary disease in NTM pulmonary disease [6] . The particularity of the CT image of the RMPL phenotype should always direct the attention to NTM disease as an important differential diagnosis.…”
