Psychogenic nonepileptic seizures (PNES) are defined by episodes of abnormal movement, sensations, or cognitive experiences similar to epileptic seizures. These events, however, are not related to abnormal electrical brain discharges and are thought to be cause by a psychological process. Children appear to carry a lower risk for PNES compared to adults and yet this diagnosis maybe present in up to 10% of the pediatric patients. The main features of PNES in such age group is discussed.Key words: epilepsy, nonepileptic seizures, nonepileptic psychogenic seizures, nonepileptic psychogenic seizures in children. RESUMO Crises não-epilépticas psicogênicas em crianças e adolescentes com epilepsiaCrises não-epilépticas psicogênicas (CNEP) são definidas como episódios de movimentos anormais, sensações ou experiências cognitivas similares a crises epilépticas. Estes eventos, contudo, não são relacionados a descargas elétricas cerebrais anormais e parecem estar associadas a processos de natureza psicológica. CNEP são menos freqüentes em crianças quando comparados a adultos, porém ainda assim este diagnóstico pode ser observado em 10% da população de pacientes pediátricos. As principais características de CNEP nestes grupo etária são discutidas nesta revisão.Unitermos: epilepsia, crises não-epilépticas, crises não-epilépticas psicogênicas, crises não-epilépticas psicogênicas em crianças.

Purpose of review: Neurologic symptoms due to a psychogenic cause are frequently seen in clinical practice. Psychogenic nonepileptic seizures (PNES) and psychogenic movement disorders (PMD) are among the most common psychogenic neurologic disorders. PNES and PMD are usually investigated and managed separately by different neurology subspecialists. We review the main epidemiologic and clinical features of both PNES and PMD, aiming to highlight their similarities and differences and to see whether a common framework for these disorders exists. Recent findings:Data from the literature show that there is a profound overlap between PNES and PMD, which would argue for a larger unifying pathophysiology with variable phenotypic manifestations. Summary: Collaborative and integrated research among epileptologists, movement disorders experts, psychiatrists, psychologists, and physiotherapists may increase our collective knowledge about the pathophysiologic mechanisms of PNES and PMD and therefore improve outcomes for these patients. Neurol Clin Pract 2016;6:138-149 N eurologic symptoms due to a functional or psychogenic cause are common in clinical practice, accounting for up to 16% of patients presenting to neurology outpatient clinics, depending on the clinical definitions and methodology used for case ascertainment.1,2 The yearly costs of patients with medically unexplained symptoms in the United States have been estimated at approximately $256 billion per year.3 Despite their prevalence and associated economic and social burden, appropriate treatments for these disorders are an unmet need. This may reflect the absence of research interest and lack of discourse between neurology and psychiatry regarding these patients. Notably, although approximately 50% of the patients with somatoform disorders are identified by neurologists at the time of first contact, only 1.5% are referred to a psychiatrist or a psychologist.

Discute o desenvolvimento do conhecimento psiquiátrico sobre a crise não epiléptica psicogênica (CNEP), buscando iluminar os pressupostos epistemológicos do conceito e suas implicações práticas. Em sua definição atual, crises ou ataques recorrentes são manifestações comportamentais semelhantes às da epilepsia, mas que dela diferem por não serem consequentes de descargas elétricas cerebrais anormais, podendo ter origem psicogênica. Em direção inversa da metafísica, investiga-se a emergência histórica do conceito de CNEP nos últimos quarenta anos. Conceitos de comorbidade psiquiátrica, abuso e dissociação foram discutidos devido a sua participação na trajetória conceitual de CNEP.
