Pruritus may precede abnormal liver function tests in pregnant women with obstetric cholestasis: a longitudinal analysis

Abstract: Obstetric cholestasis is a liver disorder unique to pregnancy, which typically presents with pruritus. However, pruritus is common in pregnancy and the diagnosis of obstetric cholestasis is confirmed by finding abnormal liver function. We report 10 cases in which pruritus occurred before any abnormality in liver function tests (including total serum bile acids) and discuss the implications of this for clinical practice.

“…The chief complaint is usually pruritus, which is classically described as total-body itching, localizing to the palms of the hands and the soles of the feet, with a nocturnal predominance. [24][25][26] Often the pruritus will precede any other physical or lab abnormalities. 24 The pruritus generally improves shortly after delivery, but is reported to recur in 40% to 60% of subsequent pregnancies.…”

“…Bei jeder Schwangeren mit Pruritus sind die Leberwerte zu bestimmen. Da der Pruritus der Leberwerterhöhung mehrere Wochen vorausgehen kann [15], sind wöchentliche Verlaufskontrollen auch bei initial normalen Laborwerten sinnvoll. Die Gallensäurenkonzentration im Serum hat als sensitivster Parameter für eine intrahepatische Cholestase besondere Bedeutung [2,9].…”

“…Although pruritus can precede any abnormalities in liver function [5], liver tests (transaminases, bilirubin) are to be performed in every pregnant woman who experiences pruritus. The diagnosis of cholestasis of pregnancy is based on otherwise unexplained pruritus and elevated serum bile acid concentrations (>10 mol/L).…”
