Proximal Opening Wedge Osteotomy With Distal Chevron Osteotomy of the First Metatarsal for the Treatment of Moderate to Severe Hallux Valgus

Abstract: Background: The aim of this study was to assess radiographic and clinical outcomes after double osteotomy with proximal opening wedge first metatarsal osteotomy and first metatarsal distal chevron osteotomy in the treatment of moderate to severe hallux valgus. Methods: 33 patients (4 male, 29 female; 36 feet; average age 60.7 years) were included in the study. Radiographic and clinical outcome in terms of intermetatarsal angle (IMA), hallux valgus angle (HVA), distal metatarsal articular angle (DMAA), and the … Show more

“…Kaufmann et al conducted a study with a large case series consisting of 524 patients and demonstrated that there was correction loss after month 3 following chevron osteotomy [24] . In a recent study conducted on 36 patients, Braitoet al applied POWO and DCO procedures together and they reported high complication rates although this approach was highly successful in correcting the deformity [25]. One of the strengths of this study was the comparison of patients included in the study in accordance with the inclusion and exclusion criteria with patients who had similar demographic and clinical characteristics.…”

“…A recent randomized controlled trial pointed out no statistically significant differences between MIIND and Reverdin-Isham percutaneous osteotomy in the treatment of mild to moderate HV, providing both good to excellent outcomes. 21 However, distal metatarsal osteotomies, 10,19,47-49 particularly the intra-articular ones, such as Reverdin-Isham, 3,6 can lead to a greater risk of I-MTPJ stiffness, probably due to the need to perform LR. On the contrary, the MIIND, applied after an extra-articular osteotomy and without LR, reduces this risk.…”

“…Among the 130 described procedures, there is no true consensus and the optimal technique has not been determined. The incorrect orientation of the articular surface of the 1 st metatarsal in juvenile hallux valgus leads us to recommend mainly techniques with osteotomy of the M1 base or cuneiform bone combined with osteotomy of the neck of the 1 st metatarsal to reorient the articular surface 35 . In the literature, a Scarf osteotomy combined as needed with an osteotomy of the 1 st phalanx (Akin 34 ) and a Chevron-type osteotomy of the neck of the 1 st metatarsal have been described extensively.…”
