Protein-losing Enteropathy: Diagnosis with99mTc-labeled Human Serum Albumin Scintigraphy

Abstract: (99m)Tc HSA with serial scanning for up to 24 hours is reliable and useful for imaging PLE. Sites of protein loss may also be demonstrated. This imaging method is convenient, easy to perform, and yields results within 24 hours.

“…CT scan in our patient was normal and scintigraphy was normal at a tertiary care centre (perhaps due to intermittent nature of protein loss) 11 . Endoscopic biopsy of duodenum, jejunum showed presence of dilated lacteals which confirmed intestinal lymphangiectasia.…”

“…Furthermore, 99mTc HSA scintigraphy revealed no accumulation of tracer in the small bowel. Considering that no obvious small bowel disease was present, collagenous colitis itself might have been the cause of PLE and 99mTc HSA scintigraphy is useful in this case [6,7] . Although the mechanisms of enteric protein loss in patients with collagenous colitis have not been clarified, abnormalities of the surface epithelium [8] , superficial capillaries [9] and pericryptal fibroblasts [10] have been hypothesized.…”

“…Low serum protein (1.75-3.47 g/dL) is termed hypoproteinemia, which is linked to liver diseases such as chronic hepatitis and cirrhosis [63]. Therefore, the precise and accurate detection of HSA is important.…”
