Burkholderia cepacia prosthetic valve endocarditis (PVE) is extremely rare, with few cases in the literature. A report of a patient with PVE is described, followed by a literature review on B. cepacia PVE. A 38 year old man with poor dentition and a history of intravenous drug use (IVDU) and mitral valve replacement was found to have a mitral valve vegetation. Five sets of blood cultures on different days grew B. cepacia. Individual sets of blood cultures on different dates also isolated S. viridans (outside hospital culture), methicillin-resistant S. epidermidis (hospital day 1), and Bacillus spp. (hospital day 6). He was successfully treated with ceftazidime and levofloxacin as dual therapy for B. cepacia PVE, in addition to vancomycin for gram positive coverage. This case report and review highlights the possibility of B. cepacia PVE in immunocompetent patients with poor dentition, with the potential for a successful outcome following combination antimicrobial therapy.

Introduction. Infectious endocarditis (IE) caused by microorganisms Burkholderia cepacia is a very rare and poorly characterized form of endocarditis.Material and methods. We observed a case of late prosthetic mitral valve IE caused by Burkholderia cepacia in a 34-year-old patient.Results. A patient with a congenital ventricular septum defect underwent surgery on the heart three times in past, including the resection of mitral valve vegetations due to IE at age 17 and mitral valve replacement plus ventricular septum defect plastic reconstruction at 33 years old. The last was complicated by postoperative purulent sterno-mediastenitis treated by thoracoplasty. 10 months later the fever with chills appeared again, a large vegetation on a mitral valve prosthesis was revealed, and Burkholderia cepacia bacteremia with multidrug resistance to antibiotics was find. After the start of treatment with trimethoprim / sulfamethoxazole, normal temperature was observed, but the course of IE was complicated by thromboembolism with a fatal outcome.Conclusions. Multidrug resistance of the pathogen to antibiotics, including those empirically prescribed for IE, is the main risk factor for the adverse outcome of IE caused by Burkholderia cepacia. The lack of generally accepted recommendations determining the doses of antibiotics prescribed in accordance with the microorganism sensitivity is the problem still present in Burkholderia cepacia IE treatment.

Advances in medical technology, an increasingly aging population, increases in clinical indications, and expanded medical insurance coverage for these devices have all contributed to this trend. Infection is considered to be one of the most serious complications of CIEDs and carries a significant risk of morbidity and mortality. Although infection with Staphylococcus sp. accounts for the majority of cases, other bacteria have been implicated as causative agents of infection of CIEDs.Case Report:We report the first case of primary pacemaker generator pocket and lead infection due to Burkholderia cepacia (B. cepacia) in the state of Qatar. To our knowledge, there have been few cases of CIED infection due to B. cepacia previously reported in the literature.Conclusions:This case raises awareness of B. cepacia as a potential opportunistic pathogen in CIED infection. The more rare bacteria require culture on special media to provide an early diagnosis to enable proper antimicrobial therapy to commence. Adherence to infection control standards during CIED insertion would reduce infection from B. cepacia.
