Prosthetic Rehabilitation and Management of an MTA‐Treated Maxillary Central Incisor with Root Perforation and Severe Internal Resorption

Abstract: Root canal perforation and root resorption are challenging clinical conditions to correctly diagnose and treat, especially when they occur in anterior teeth. This clinical report describes the computed tomography findings, endodontic treatment, prosthetic rehabilitation, and clinical outcome of an iatrogenic root perforation and internal resorption in a maxillary central incisor. The case management consisted of endodontic retreatment, periodontal surgery, and prosthetic rehabilitation. Gray mineral trioxide a… Show more

“…O restabelecimento do ligamento periodontal depende grandemente da regeneração do cemento sobre o defeito da raiz. Uma vez que o cemento cobriu o local reparado, o restabelecimento do ligamento periodontal é mais previsível 10 .Tendo em vista que perfurações de furca podem ter um impacto negativo no prognóstico por comprometer o aparelho de fixação através de inflamações que podem levar à perda óssea 14,15 .…”

“…The management of root resorption can include conservative or surgical treatment, depending on the location, degree, and extent of occurrence [ 272 ] ( Figure 13 ). Root resorption and perforation appear together in many cases, and MTA used in their treatment has been reported to have satisfactory long-term results [ 273 , 274 , 275 , 276 , 277 , 278 , 279 , 280 ].…”

“…Root canal treatment is the indicated approach for internal root resorption (IRR) when the affected teeth is restorable and have a reasonable prognosis (1). Many cases have reported successful treatment for anterior and posterior teeth with radicular perforations and communication with the periodontal tissues (2)(3)(4)(5). The IRR promotes defects that usually renders the unachievement of a complete bio-mechanical preparation; since, some parts of the lesion remain untouched during canal instrumentation and require adjunctive chemical debridement techniques, such as, passive ultrasonic irrigation (PUI) to improve the effectiveness of the root canal irrigants (6).…”
