Prospective Evaluation of Lesion Index-Guided Pulmonary Vein Isolation Technique in Patients with Paroxysmal Atrial Fibrillation: 1-year Follow-Up.

Abstract: In this first prospective study, LSI-guided PVI improved clinical outcome without any increase in complication rate when compared with standard, non-LSI-guided approach.

“…For TACTICATH catheter, the contact force targeted before lesion delivery is 10 to 40 g, with a minimum individual lesion duration of 400 g-seconds-force-time integral. A target lesion size index of 5.0 to 5.5 at posterior wall and 5.5 to 6.0 at anterior and septal wall was achieved in all of our patients [19] , [20] . For SMARTTOUCH catheter, ablation index was used (550 for anterior and roof and 400 for posterior and inferior LA segments [21] , [22] , [23] .…”

“…LSI incorporates CF between the tissue and the ablation tip, current, and duration of RF application as well as a resistive and conductive tissue-heating component. For a detailed description of LSI, see Mattia et al 12,17 Pig hearts were obtained from a local butcher and placed in a chamber containing physiologic saline solution at room temperature. The solution was circulated using an external pump at a rate of 5 L/min.…”

“…There are many parameters that can guide RF ablation during an AF ablation such as a reduction in bipolar voltage amplitude of the near-field electrogram, noncapture of the underlying myocardium, 9 predefined drops in impedance, and use of proprietary algorithms like force-time integral, Visitag R , Ablation Index R , and Lesion size index R . 15,16 However, none of these parameters have been validated for HPSD RF energy application. Modification of unipolar waveform by means of the complete elimination of the negative component may serve as an end-point for HPSD lesions.…”
