Prospective comparative study of endoscopic submucosal dissection and gastrectomy for early neoplastic lesions including patients’ perspectives

Abstract: In patients with early gastric neoplasia, ESD is safer and is associated with a positive impact on health-related QoL when compared with gastrectomy, without increasing fear of recurrence and new lesions.

“…This was externally validated by the same group, and for a diagnosis of OLGIM III/VI the AUC was 0.96 (95% CI 0.93 to 0.98). This endoscopic grading of GIM was 89% sensitive and 95% specific for a risk stratification of moderate to severe GIM if a cut-off score of >4 was used 172. On this basis, it could be argued that endoscopic staging with high-resolution WLE plus NBI is sufficiently accurate for diagnosis and staging.…”

“…This minimally invasive treatment approach not only offers an excellent prognosis but also substantially improves the quality of life of patients with EGC. 2,3 Despite the above, ESD is a highly demanding technique with a higher risk of perforation than standard EMR methods. 4 It requires a fully skilled endoscopist to ensure success and reduce adverse events; this entails specific training and an appropriate learning curve.…”

“…The main advantage of ESD relies in its ability to achieve en‐bloc instead of piecemeal resection regardless of the size of the lesion or presence of fibrosis. This minimally invasive treatment approach not only offers an excellent prognosis but also substantially improves the quality of life of patients with EGC 2,3 …”

“…Although these programs can also be cost-effective in intermediate risk countries [59,60], they have still not been implemented, and current recommendations suggest the diagnosis and surveillance of individuals with extensive preneoplastic conditions through the use of IEE [59,[61][62][63]. This approach allows the possibility of offering an endoscopic treatment instead of gastric surgery, so avoiding the associated morbidity and mortality [59,62,64,65]. According to the IEE technology being used, descriptors for preneoplastic conditions or EGC have been modified over time.…”
