METHODS. Volume optical coherence tomography (OCT) scans from 114 eyes of 114 patients were retrospectively reviewed. HRF were assessed both qualitatively and quantitatively. Five sequential en face slabs from midretina were thresholded to isolate the HRF. These five slabs were recombined, and HRF area was measured in the whole 6 3 6-mm image (HRF TOT ) and within the central 3-mm (HRF 3mm ) and 5-mm (HRF 5mm ) regions. These measurements were correlated with the development of late AMD (defined as choroidal neovascularization [CNV] and/or complete RPE and photoreceptor atrophy [cRORA]) after 1 year of follow-up.RESULTS. HRF area in all three regions showed significant correlations with progression to late AMD: R ¼ 0.610 for HRF 3mm , R ¼ 0.622 for HRF 5mm , and R ¼ 0.614 for HRF TOT (all P < 0.001). Correlations remained significant with progression to cRORA alone, though not for progression to CNV alone. While qualitative assessment of HRF (i.e., presence of HRF: yes or no) also showed a significant correlation with progression to late AMD (R ¼ 0.454, P < 0.001) and atrophy alone (R ¼ 0.445, P < 0.001), they were weaker than by HRF quantification.CONCLUSIONS. The area of HRF from en face OCT in eyes with intermediate AMD correlates with the 1-year risk of progression to late AMD, and in particular with the development of atrophy.

IMPORTANCE Risk factors associated with the development of neovascular age-related macular degeneration (AMD) have been identified. However, population size and methods to integrate imaging, genetic, and demographic factors associated with conversion to neovascular AMD are limited, specifically when treatment is administered in 1 eye. OBJECTIVE To determine the imaging, genetic, and demographic factors associated with conversion from nonneovascular to neovascular AMD in fellow eyes. DESIGN, SETTING, AND PARTICIPANTS This post hoc secondary analysis of the 24-month phase 3 multicenter, double-masked, active treatment-controlled HARBOR trial included 686 fellow eyes with nonneovascular AMD at baseline. Imaging features describing the presence, number, extent, density, and relative reflectivity of drusen were automatically extracted from spectral-domain optical coherence tomography scans. Genetic analysis included 34 single-nucleotide polymorphisms. Least absolute shrinkage and selection operator regression was performed to narrow imaging features. Survival analysis and Cox proportional hazards regression were performed to determine the association of the selected imaging features and genetic and demographic factors with conversion to neovascular AMD.

To develop a convolutional neural network visualization strategy so that optical coherence tomography (OCT) features contributing to the evolution of age-related macular degeneration (AMD) can be better determined. We have trained a U-Net model to utilize baseline OCT to predict the progression of geographic atrophy (GA), a late stage manifestation of AMD. We have augmented the U-Net architecture by attaching deconvolutional neural networks (deconvnets). Deconvnets produce the reconstructed feature maps and provide an indication regarding the inherent baseline OCT features contributing to GA progression. Experiments were conducted on longitudinal spectral domain (SD)-OCT and fundus autofluorescence images collected from 70 eyes with GA. The intensity of Bruch's membrane-outer choroid (BMChoroid) retinal junction exhibited a relative importance of 24%, in the GA progression. The intensity of the inner retinal pigment epithelium (RPE) and BM junction (InRPEBM) showed a relative importance of 22%. BMChoroid (where the AMD feature/damage of choriocapillaris was included) followed by InRPEBM (where the AMD feature/damage of RPE was included) are the layers which appear to be most relevant in predicting the progression of AMD.
