Propofol anaesthesia and postoperative nausea and vomiting: quantitative systematic review of randomized controlled studies.

Abstract: We have analysed randomized controlled studies which reported the incidence of postoperative nausea and vomiting (PONV) after propofol anaesthesia compared with other anaesthetics (control). Cumulative data of early (0-6 h) and late (0-48 h) PONV were recorded as occurrence or non-occurrence of nausea or vomiting. Combined odds ratio and number-needed-to-treat were calculated for propofol as an induction or maintenance regimen, early or late outcomes, and different emetic events. This was performed for all con… Show more

“…The direction of the treatment effect in favour of propofol ⁄ propofol is supported by the literature. PONV incidence in the trial is at the low end of the reported range [19][20][21][22]. The direction of the treatment effect in favour of propofol ⁄ propofol is supported by the literature.…”

“…It is not useful for PONV prophylaxis if given only as a bolus for induction of anesthesia (IIA). 36 More recently, continuous subhypnotic propofol infusion and the use of patient-controlled antiemesis with propofol were also found to be effective in the treatment of PONV (IIIA). 86,87 The effective plasma concentration of propofol for the 50% reduction in nausea scores has been found to be 343 ng·mL -1 .…”

“…15 TIVA with propofol has been shown to reduce the incidence of PONV, especially in the early postoperative period. 36 Avoidance of nitrous oxide (which increases postoperative vomiting) and volatile agents (which cause PONV for up to two hours postoperatively), and minimizing intraoperative and postoperative opiates, also reduce the incidence of PONV. 2,[8][9][10][11][12][13] The use of large doses of neostigmine (> 2.5 mg) increases the risk of PONV.…”

“…12 In most meta-analyses, propofol was associated with a lower frequency of PONV when used for total iv anesthesia in the absence of N 2 O. [12][13][14][15] In one meta-analysis, the rate of PONV was lower with the use of any propofol when compared with sevoflurane. 16 The omission of anticholinesterases (for reversal of muscle relaxation) also reduced PONV but also resulted in an increased risk of clinically relevant residual muscle weakness.…”
