Prophylaxis of Experimental Endocarditis With Antiplatelet and Antithrombin Agents: A Role for Long-term Prevention of Infective Endocarditis in Humans?

Abstract: Antiplatelet and direct antithrombin agents may be useful in the prophylaxis of IE in humans. In particular, the potential dual benefit of dabigatran etexilate might be reconsidered for patients with prosthetic valves, who require life-long anticoagulation and in whom S. aureus IE is associated with high mortality.

“…Platelets play a pivotal role in the progression and development of IE, but their role can be likened to a double-edged sword (185)(186)(187)(188). Platelets display both haemostatic and host defence properties.…”

“…The ability of sera from immunized rats to inhibit S. gordonii-induced platelet aggregation was evaluated in vitro by conventional light aggregometry as previously described (29). Briefly, 20 l of S. gordonii (final concentration of 1 ϫ 10 9 CFU/ml in Tyrode's buffer) was preincubated for 1 h at room temperature with 2 l of control or anti-Hsa-, anti-PadA-, and anti-Hsa-plus anti-PadA-containing sera and then added to 180 l of PRP in siliconized flat-bottom cuvettes.…”

“…99 It was recently shown that treatment with the thrombin and staphylothrombin inhibitor dabigatran could protect against the development of S. aureus endocarditis in rats. 100 Establishment of IE requires adhesion to the vessel wall under high shear stress conditions. The interaction of S. aureus vWbp with VWF, either attached to the subendothelium or released by inflamed endothelial cells, is sufficient to mediate adhesion in flow conditions in vivo.…”
